Summary: This study analyzed the outcomes and risk factors of intraoperative conversion during video-assisted thoracoscopic surgery (VATS) and robotic lobectomy (RL), and found that RL had a lower conversion rate compared with VATS.

Summary: Conversion to thoracotomy during minimally invasive lobectomy is occasionally necessary. Differences between video-assisted thoracoscopic surgery (VATS) and robotic-assisted thoracoscopic surgery (RATS) lobectomy conversion have not been described.

Patient risk factors for conversion during video-assisted thoracic surgery-the Epithor conversion score

Summary: The study aims to identify risk factors for intraoperative conversion during anatomical lung resection and develop a scoring system. Through a retrospective analysis of a multicenter database, 13 potential risk factors were identified and an easy-to-use scoring tool was developed.

Risk factors of conversion in robotic- and video-assisted pulmonary surgery for non-small cell lung cancer

Dali Chen et al.

Summary: This study showed that the robotic approach significantly reduced the risk of conversion to thoracotomy for NSCLC patients compared to VATS. Factors such as robotic approach, tumor size < 5cm, absence of chief symptoms, body mass index < 25 kg/m2, and lobectomy were identified as independent protectors against conversion.

Intraoperative conversion during video-assisted thoracoscopy resection for lung cancer does not alter survival

Alex Fourdrain et al.

Summary: This study assessed the long-term outcomes of patients treated for lung cancer with VATS, including those who required intraoperative conversion to thoracotomy. The study found that intraoperative conversion did not significantly impact long-term oncological outcomes compared to full-VATS or open upfront thoracotomy. Postoperative complications were lower in the full-VATS group and similar between the VATS-conversion and thoracotomy groups, suggesting VATS should be preferred when possible.

Conversion from video-assisted thoracic surgery (VATS) to thoracotomy during major lung resection: how does it affect perioperative outcomes?

Joseph Seitlinger et al.

Summary: The global conversion rate of VATS lung resection has decreased to less than 10% over the years. However, patients who undergo conversion to thoracotomy have a higher risk of complications (40.9% vs 16.8%) and perioperative mortality (6.8% vs 0.2%) compared to those who complete VATS procedure.
