A 24-year-old patient presented with complete inability to chew food on the left side of the mouth. Clinical investigation showed a complex malformation of the maxilla with inclined masticatory surface, tooth dysplasia, and eruption failure, resulting in an open bite on the left side. The diagnosis of primary failure of eruption was established. This article presents the clinical picture, surgical management, and further clinical course.
