Radial wall strain: a novel angiographic measure of plaque composition and vulnerability

Summary: The angiography-derived radial wall strain (RWS) was found to be associated with plaque composition and vulnerability features as assessed by optical coherence tomography (OCT). In patients with intermediate coronary stenosis, RWS was positively correlated with lipid plaque burden and the thinnest fibrous cap thickness, while negatively correlated with fibrous cap thickness. An RWSmax >12% could serve as an angiographic predictor for lipid-to-cap ratio (LCR) >0.33 and thin-cap fibroatheroma (TCFA).

Short-Term Risk Stratification of Non-Flow-Limiting Coronary Stenosis by Angiographically Derived Radial Wall Strain

Summary: This study aimed to evaluate the incremental value of angiography-derived radial wall strain (RWS) in risk stratification of non-flow-limiting mild coronary narrowings. The results showed that RWSmax >12% was an independent predictor of deferred revascularization, and its combination with Murray-law-based quantitative flow ratio (mQFR) significantly reduced the risk of deferred revascularization.

Summary: In patients with STEMI undergoing complete revascularization, an FFR-guided strategy did not have a significant benefit over an angiography-guided strategy with respect to the risk of death, myocardial infarction, or urgent revascularization at 1 year. Summary: This study aimed to investigate the impact of optical coherence tomography (OCT)-detected thin-cap fibroatheroma (TCFA) on clinical outcomes of diabetes mellitus (DM) patients with fractional flow reserve (FFR)-negative lesions. The results showed that among DM patients with FFR-negative lesions, TCFA-positive patients had a five-fold higher rate of major adverse clinical events (MACE) compared to TCFA-negative patients, suggesting a potential paradigm shift for coronary artery disease risk stratification in DM patients.
