A multiancestry genome-wide association study of unexplained chronic ALT elevation as a proxy for nonalcoholic fatty liver disease with histological and radiological validation

Marijana Vujkovic et al.

Summary: Through a multi-ancestry genome-wide association study, we identified multiple genetic loci associated with NAFLD in the Million Veteran Program, some of which are also related to metabolic and/or inflammatory traits. Integrating cALT, histology, and imaging provided new insights into genetic susceptibility to NAFLD.

To curb research misreporting, replace significance and confidence by compatibility A Preventive Medicine Golden Jubilee article

Sander Greenland et al.

Summary: Statistical analyses in health-science and medical journals are often misleading or wrong. Efforts to reform the problem have not been successful, and we propose replacing traditional jargon with more accurate and modest ordinary-language labels to describe statistics as measures of compatibility between data and hypotheses or models. Additionally, the introduction of surprisal or S-values can provide an understanding of the amount of information the data supply against these possibilities.

Artificial Intelligence for Detecting and Quantifying Fatty Liver in Ultrasound Images: A Systematic Review

Fahad Muflih Alshagathrh et al.

Summary: Non-alcoholic Fatty Liver Disease (NAFLD) is increasing worldwide, and the efficacy of traditional diagnostic methods such as ultrasonography and clinical scoring systems is being questioned. Artificial Intelligence (AI) combined with traditional diagnostic processes has shown potential for improving the diagnosis and quantification of NAFLD. This systematic review found that AI-supported systems significantly enhance the diagnosis of NAFLD, Non-Alcoholic Steatohepatitis (NASH), and liver fibrosis. Prospective studies comparing AI-assisted modalities with conventional techniques are needed before real-world implementation.

Hepatocellular and extrahepatic cancers in non-alcoholic fatty liver disease: A systematic review and meta-analysis

James A. Thomas et al.

Summary: Non-alcoholic fatty liver disease (NAFLD) is associated with hepatocellular carcinoma (HCC) and extrahepatic cancer. The incidence rate of HCC is higher in patients with advanced liver fibrosis or cirrhosis, while the incidence rate of extrahepatic cancers is even higher. These findings emphasize the importance of prevention and early detection of cancer in adults with NAFLD.

Summary: The PNPLA3 rs738409 G allele is associated with NAFLD, hepatocellular carcinoma, and overall or cardiovascular mortality. Studies show that women carrying the G allele without hepatic steatosis have a 60% lower risk for cancer-related mortality.

Prospective Study of Outcomes in Adults with Nonalcoholic Fatty Liver Disease

Summary: In this study involving patients with nonalcoholic fatty liver disease, advanced fibrosis stages F3 and F4 were associated with increased risks of liver-related complications and death over a median of 4 years of follow-up. Advanced fibrosis was also related to an increased incidence of liver decompensation events.

Normalization of γ-glutamyl transferase levels is associated with better metabolic control in individuals with nonalcoholic fatty liver disease

Summary: In patients with NAFLD, those with concurrently elevated ALT and GGT levels showed a lower normalization rate of GGT rather than ALT. Good control of weight and insulin resistance was a reliable predictor of GGT normalization.

Mortality in Patients With Genetic and Environmental Risk of Liver Disease

Carolin V. Schneider et al.

Summary: This study analyzed data from a large sample recruited by the UK Biobank and found that genetic factors have a greater impact on liver-specific mortality, while certain gene mutations contribute to overall mortality. In addition, obesity and metabolic syndrome disproportionally contribute to liver-related deaths. The findings highlight the interplay between genetics and environment in liver-related mortality and provide a basis for hepatic surveillance programs.

Clinical Care Pathway for the Risk Stratification and Management of Patients With Nonalcoholic Fatty Liver Disease

Fasiha Kanwal et al.

Summary: NAFLD is a common disease that affects a significant proportion of US adults. Management typically occurs in primary care or endocrine clinics, where accurate staging for fibrosis risk through screening is crucial, requiring consistent, timely, evidence-based, and easily testable management processes.

Summary: NAFLD is a major public health issue globally, with its burden expected to increase. Patients with T2DM are at high risk for NASH and advanced fibrosis, urging for NAFLD screening. A simple algorithm can efficiently identify high-risk individuals in primary care or diabetology clinics.
