'Paper care not patient care': Nurse and patient experiences of comprehensive risk assessment and care plan documentation in hospital

Summary: This study aims to explore organization-wide experiences of person-centred care and risk assessment practices. The findings suggest that the structure of the documentation is efficient and valued by nurses, and the ISBAR framework and clinical decision-making prompts are important. However, there are challenges in obtaining patient signatures, multidisciplinary involvement, duplication of paperwork, and capturing person-centred goals in care plan documentation. Patient satisfaction with person-centred care is high, but there are issues with communication and integration of care within the multidisciplinary team.

Does implementation of a tailored intervention increase adherence to a National Safety and Quality Standard? A study to improve delirium care

Summary: This study aimed to evaluate the efficacy of a delirium prevention intervention within an orthopaedic care system. Results showed a significant decrease in the rate of delirium, but no significant changes in duration or hospital stay length. The success of the project can be attributed to the use of education, environmental changes, and audit feedback as intervention strategies.

Characterising the nature of clinical incidents reported across a tertiary health service: a retrospective audit

Summary: This study analyzed patient safety incidents in a tertiary health service in Australia and found that the most common incidents were related to skin injury, medication, falls, and clinical process. There were empirical associations between the type of PSI and clinical division (medical, surgical), which may inform more targeted quality improvement strategies to reduce their incidence.
