Hepatic fibrosis: 2022 Unmet needs and a blueprint for the future

Summary: Steady progress has been made in understanding the pathogenesis and clinical consequences of hepatic fibrosis, but effective antifibrotic drugs are yet to be approved. This article highlights the successes and identifies gaps and unmet needs in both experimental and clinical aspects. Clarifying the mechanisms of fibrosis regression and developing better models will expedite drug development, while improving diagnostics and utilizing digital pathology methods can enhance prognostic information and better quantify therapeutic responses.

Summary: In this study, a high-fat choline diet-induced atherosclerosis model was established in ApoE(-/-) mice to investigate the anti-atherosclerotic effects of gypenoside XLIX (GPE). The results showed that GPE treatment could reduce dyslipidemia, atherosclerotic plaque, inflammation, and gut microbiota dysfunction induced by the high-fat diet. The changes in intestinal microbiota and anti-inflammatory activity were found to be related to the anti-atherosclerotic effect of GPE.

Additionally, NF-kappa Bp65 in hepatocytes repressed PUMA-mediated hepatocyte apoptosis by regulating the Bcl-2 family, and NF-kappa Bp65 deficiency in hepatocytes enhanced apoptosis-linked inflammatory response, contributing to the activation of HSCs and liver fibrogenesis. These findings suggest that Fas/FasL contributes to NF-kappa Bp65/PUMA-modulated hepatocyte apoptosis via autophagy to enhance liver fibrogenesis, providing a potential therapeutic target for liver fibrosis.
