Maternal age affects pronuclear and chromatin dynamics, morula compaction and cell polarity, and blastulation of human embryos

Kenji Ezoe et al.

Summary: Maternal ageing significantly affects pronuclear size and intra-and extra-nuclear dynamics during fertilization, dysregulates cell polarity during compaction, and reduces blastocoel expansion.

Metaphase-II oocyte competence is unlinked to the gonadotrophins used for ovarian stimulation: a matched case-control study in women of advanced maternal age

Alberto Vaiarelli et al.

Summary: This study aimed to explore the impact of different gonadotrophin selection for ovarian stimulation on oocyte competence. The results showed that the choice of recombinant FSH or human menopausal gonadotrophin for ovarian stimulation was associated with the euploid blastocyst rate. However, there was no association between the type of gonadotrophin used and live birth rate or blastocyst quality.

Cell-Free Fetal DNA and Non-Invasive Prenatal Diagnosis of Chromosomopathies and Pediatric Monogenic Diseases: A Critical Appraisal and Medicolegal Remarks

Giuseppe Gullo et al.

Summary: Cell-free fetal DNA (cffDNA) analysis is a non-invasive prenatal diagnostic test used to screen for chromosomal or monogenic pathologies of the fetus. A narrative review of 45 studies was conducted to critically assess its diagnostic value, and it was found to be well-established for screening purposes. The review also highlighted the need for validation studies and discussions involving all stakeholders to address the legal and ethical complexities associated with these techniques.

Summary: This review discusses the mechanisms and regulatory pathways that govern the processes of oocyte and follicle formation, growth, and ovulation, with a focus on human oocytes/follicles. It emphasizes the effects of aging on oocyte number and quality, as well as the clinical implications. Additionally, it outlines the current developments in culture systems for follicle growth and the emerging prospects for generating new oocytes from stem cells.

Summary: The success of an assisted reproduction cycle depends on selecting the best embryo with high implantation potential. Traditional morphological evaluation methods have limitations, but the introduction of time-lapse monitoring technology allows dynamic observation of embryo development and has been widely used in IVF clinics worldwide.

The destinies of human embryos reaching blastocyst stage between Day 4 and Day 7 diverge as early as fertilization

Summary: Comparing embryos reaching the blastocyst stage between Days 4-7, it was found that longer times to blastocyst formation are associated with worse clinical outcomes and disruptions in developmental patterns. These differences emerge as early as the fertilization stage.

Oocytes evaluation and in-vitro fertilization/ intra cytoplasmic sperm injection outcomes

Maria Giulia Minasi et al.

Summary: There are currently no definitive criteria for evaluating the quality and competence of oocytes. Advanced maternal age is a major factor leading to low oocyte quality, and other factors such as obesity, lifestyle, genetic and systemic pathologies, ovarian stimulation protocols, laboratory procedures, culture, and environmental conditions may also influence oocyte competence. Morphological and maturational evaluation of oocytes is the most commonly used method. Other criteria such as cumulus cell abnormalities and organelle aggregation are also associated with embryo developmental potential. However, many evaluation methods are still limited to research and have not been widely applied in clinical practice. Therefore, oocyte morphology and maturity remain important indicators for assessing oocyte quality.

A long-acting recombinant FSH supports high-quality mouse follicle development and oocyte maturation in vitro by coordinating somatic and germ cell transcriptomes

Shao-Yuan Liu et al.

Summary: In this study, a new type of recombinant FSH (KN015) with a prolonged half-life is proposed as an alternative to canonical FSH. KN015 supports the in vitro development of mouse follicles from primary to preovulatory stage with higher efficiency than canonical FSH and enhances post-fertilization development rates of the ovulated oocytes. The findings of this study provide new transcriptome data for oocyte and granulosa cells at the key stages of follicle development, and should help to widen the use of KN015 as a valid and commercially available hormonal support enabling optimized in vitro development of follicles and oocytes.

Why double ovarian stimulation in an in vitro fertilization cycle is potentially unsafe

Angelo Tocci

Summary: This study found that two antral follicle recruitment waves occur in the ovaries of normal women, supporting the use of double ovarian stimulation in a single cycle to benefit poor responder patients. However, the mechanism and safety of the double stimulation strategy in the same cycle have been poorly addressed in previous studies.

Mild/moderate versus full stimulation

Carlo Alviggi et al.

Summary: Mild stimulation has gained popularity in assisted reproduction due to its reduced costs and reduced risk of hyperstimulation syndrome. However, using mild stimulation in women with low prognosis has risks such as higher cycle cancellation rate and reduced number of eggs collected. Despite this, collecting an adequate number of eggs remains crucial for improving live birth rates.

Regulation of antral follicular growth by an interplay between gonadotropins and their receptors

Livio Casarini et al.

Summary: Knowledge of human antral follicle growth and maturation is primarily based on clinical observations and animal models. New experimental approaches and in vitro data have contributed to a better understanding of gonadotropin receptor functioning, shedding light on unclear physiological events. A new hypothesis based on receptor-receptor interactions explains the molecular mechanisms of antral follicle growth, mediating steroidogenesis, apoptotic events, and dominance.

Recombinant human luteinizing hormone co-treatment in ovarian stimulation for assisted reproductive technology in women of advanced reproductive age: a systematic review and meta-analysis of randomized controlled trials

Summary: Studies suggest that LH can improve IVF outcomes in older women by optimizing androgen production. A meta-analysis found that r-hFSH/r-hLH co-treatment was more effective in improving clinical pregnancy and implantation rates in women aged 35-40, despite retrieving fewer oocytes compared to r-hFSH monotherapy. More research is needed to confirm these findings.
