Tumor Microenvironment in Sporadic Vestibular Schwannoma: A Systematic, Narrative Review

Summary: Although there have been improvements in the diagnosis and treatment of vestibular schwannomas (VSs) in recent years, no factors have been identified as predictive of tumor growth. The tumor microenvironment (TME) plays a key role in tumorigenesis, drug response, and treatment outcome. This review aims to assess the current knowledge on TME in sporadic VSs, and highlights the need for further studies to identify TME-related biomarkers and therapeutic targets.

Summary: Patients presenting with unilateral vestibular schwannomas (UVS) and multiple meningiomas (MM) are more likely to be diagnosed with neurofibromatosis type 2 (NF2) compared to patients with UVS and multiple nonintradermal schwannomas (NIDS), but less likely to develop bilateral vestibular schwannomas. Some patients with MM without meeting NF2 criteria have pathogenic variants in SMARCE1 and mosaic NF2.

Surrogate vascular input function measurements from the superior sagittal sinus are repeatable and provide tissue-validated kinetic parameters in brain DCE-MRI

Summary: This study compared vascular input functions (VIF) extracted from different vascular locations and found that the arrival-corrected vascular output function (VOFSSS) derived from the superior sagittal sinus (SSS) performed better as a global input function compared to arteries. The VOFSSS showed higher contrast-to-noise ratio, peak, and sensitivity to plasma gadolinium-based contrast agent (GBCA) concentration. In addition, it allowed detection of intertumoural differences within resected tissue specimens.

Research on the Mechanism of Cognitive Decline in Patients With Acoustic Neuroma

Summary: This study compared the cognitive function of patients with acoustic neuroma (AN) and healthy controls (HCs) and explored the possible underlying mechanisms. The results showed that patients with AN performed worse in neuropsychological evaluations and had widespread alterations in different diffusion tensor imaging (DTI) metrics, which correlated with cognitive function. It was also found for the first time that the decrease in fractional anisotropy (FA) in the minor forceps was related to cognitive decline, suggesting it could be a neurobiological marker of cognitive impairment in AN patients.

Cognitive decline in acoustic neuroma patients: An investigation based on resting-state functional magnetic resonance imaging and voxel-based morphometry

Summary: This study aimed to investigate the cognitive function changes in patients with acoustic neuroma (AN) and explored possible mechanisms using structural and functional magnetic resonance imaging (fMRI). The results showed that AN patients had worse cognitive performance compared to healthy controls. Functional activity was increased in certain brain regions of AN patients, compensating for cognitive decline. Additionally, decreased functional connectivity and changes in brain structure were observed in AN patients. These findings highlight the importance of understanding cognitive impairment and brain changes in AN patients.

Brain structural network to investigate the mechanism of cognitive impairment in patients with acoustic neuroma

Summary: Patients with acoustic neuroma show cognitive impairments in various domains, including memory, executive function, attention, and processing speed compared to healthy controls. Changes in the structural brain network, especially the efficiency of subnetworks and fiber connections, are associated with cognitive impairments in these patients. Including neuropsychological aspects in clinical evaluation and appropriate treatments may help improve the life quality of patients with acoustic neuroma.
