Enhanced diagnosis of advanced fibrosis and cirrhosis in individuals with NAFLD using FibroScan-based Agile scores

Summary: This study aims to develop and validate two new scoring systems (Agile 4 and Agile 3+) to accurately identify cirrhosis or advanced fibrosis among individuals with non-alcoholic fatty liver disease (NAFLD) attending liver clinics. The results showed that these two novel non-invasive scores outperformed existing scoring systems in identifying cirrhosis or advanced fibrosis and reduced the need for liver biopsy.

Summary: Human breath offers several benefits for diagnostic applications, including simple, noninvasive collection. Breath is a rich source of clinically-relevant biological information, such as volatile organic compounds (VOCs) and aerosols carrying nucleic acids, proteins, signaling molecules, and pathogens. Breath biomarkers can advance early detection and precision medicine. Breath tests are noninvasive and can be performed at home or in a primary care setting, making them suitable for public screening programs. Measurement of VOCs on breath can help detect metabolic changes and a range of illnesses.

Early intervention can improve the natural history of these diseases, but many patients present with advanced disease. Liver function tests (LFTs) are commonly measured, but a majority of abnormal results are not further investigated according to guidelines. Recently, diagnostic pathways have been implemented in the UK, focusing on abnormal LFTs and stratifying at-risk populations.

The use of an exogenous volatile organic compounds (EVOCs) probe approach may help overcome the limitations of random dietary exposure and has shown potential in healthy subjects.
