Physiological underpinnings of exertional dyspnoea in mild fibrosing interstitial lung disease

Summary: The functional disturbances underlying dyspnoea in patients with fibrosing interstitial lung disease (f-ILD) and mild restrictive abnormalities are not well understood. In this study, patients with preserved lung function but reduced total lung capacity showed higher ventilation and dyspnoea scores during exercise compared to controls. The degree of dyspnoea was associated with lower lung diffusing capacity and exercise capacity in f-ILD patients. These findings suggest that heightened ventilatory demands and decreased tidal volume expansion explain the excessive dyspnoea in these patients.

The use of 6MWT outcomes as primary end-points should take into consideration the study population, disease progression, and expected treatment effect.
