4DCT ventilation function image-based functional lung protection for esophageal cancer radiotherapy

Pixiao Zhou et al.

Summary: This study used 4DCT to generate functional lung ventilation images and evaluate the correlation between functional lung dose-volume parameters and changes in pulmonary function metrics in patients with esophageal cancer. The study also compared the effectiveness of functional lung sparing planning versus non-sparing planning. The results showed that the optimal functional lung threshold for dose-volume parameters was strongly correlated with changes in FEV1/FVC, and functional lung sparing planning reduced functional lung dose without compromising target area coverage.

Results of a Multi-Institutional Phase 2 Clinical Trial for 4DCT-Ventilation Functional Avoidance Thoracic Radiation Therapy

Yevgeniy Vinogradskiy et al.

Summary: Functional avoidance radiation therapy reduces the occurrence of radiation pneumonitis by designing radiation therapy plans that minimize doses to functional areas of the lung. A multicenter study using 4DCT-ventilation functional imaging demonstrated that this treatment approach met the criteria for phase 2 trials, suggesting further evaluation in a phase 3 study is warranted.

Dose-volume comparison of intensity modulated proton therapy and volumetric modulated arc therapy for cervical esophageal cancer

Takahiro Kato et al.

Summary: This study compared the dosimetric characteristics of intensity modulated proton therapy (IMPT) with volumetric modulated arc therapy (VMAT) for cervical esophageal cancer. The results showed that IMPT could significantly reduce lung and spinal cord doses, as well as the integral whole body dose, while maintaining similar target coverage compared to VMAT.

Proton Therapy With Concurrent Chemotherapy for Thoracic Esophageal Cancer: Toxicity, Disease Control, and Survival Outcomes

Michael S. Rutenberg et al.

Summary: This study reports the outcomes of disease control, survival, and toxicity in patients with esophageal cancer treated with proton radiation therapy and concurrent chemotherapy. Proton radiation therapy allows for a reduced radiation dose to the heart and lungs, potentially reducing the risk of cardiopulmonary toxicity. The results demonstrate that proton radiation therapy is a safe and effective treatment for esophageal cancer with promising evidence of reduced cardiopulmonary toxicity.

Dosimetric analysis and biological evaluation between proton radiotherapy and photon radiotherapy for the long target of total esophageal squamous cell carcinoma

Yongbin Cui et al.

Summary: This study compares the differences in dosimetric and biological evaluations between photon and proton radiation therapy. The results show that proton radiation therapy is superior to photon radiation therapy in terms of the plan conformity index, homogeneity index, and gradient index. Proton radiation therapy improves tumor control probability and reduces the dose and volume of radiation received by the lungs, heart, and spinal cord compared to photon therapy.

Functional lung imaging in thoracic tumor radiotherapy: Application and progress

Pi-Xiao Zhou et al.

Summary: Radiotherapy plays a crucial role in thoracic tumor treatment, but radiation-induced lung injury limits the increase in target doses. The introduction of functional lung imaging allows for the design of a functional lung protection plan, reducing the impact of radiation on functional lung and decreasing the occurrence of radiation-induced lung injury.

The feasibility of dose escalation using intensity-modulated radiotherapy (IMRT) and intensity-modulated proton therapy (IMPT) with FDG PET/CT guided in esophageal cancer

Yiyuan Zhang et al.

Summary: This study evaluated the feasibility of dose escalation in tumor regions with high FDG uptake using IMRT and IMPT. The results showed that dose escalation by IMRT may increase the risk of radiation-related injury, while further dose escalation in high FDG uptake regions did not increase doses to organs-at-risk. This dose escalation is ideal for achieving better outcomes for EC treatment.

Health Care Resource Utilization for Esophageal Cancer Using Proton versus Photon Radiation Therapy

Steven H. Lin et al.

Summary: Proton beam therapy (PBT) reduces toxicities and postoperative complications (POCs) compared with intensity-modulated radiation therapy (IMRT) in patients with esophageal cancer (EC) treated with chemoradiation. The study suggests that PBT has higher upfront resource utilization but lower postoperative healthcare charges, especially among patients with any POCs.

Comparison of intensity modulated proton therapy beam configurations for treating thoracic esophageal cancer

Summary: This study proposed new intensity-modulated proton therapy (IMPT) beam configurations for treating esophageal squamous cell carcinoma (ESCC) and demonstrated the significant benefit of IMPT compared to intensity-modulated x-ray therapy (IMXT) in reducing radiation doses to surrounding organs. The optimal IMPT beam configuration depends on the tumor location relative to the heart, with IMPT plans showing improved organ-at-risk sparing.

Summary: Concurrent chemoradiation is crucial in treating esophageal cancer, and proton beam therapy shows promise in reducing toxicity and improving clinical outcomes. Proton beam therapy can achieve lower vertebral bone marrow dose than photon beam therapy during chemoradiation therapy of esophageal cancer

Yin Zhang et al.

Summary: This study compared the effects of proton and photon radiation therapy on vertebral body doses in chemoradiation for esophageal cancer patients, finding that proton therapy can better spare vertebrae and avoid myelosuppression. This has implications for avoiding delays or dose reductions in chemotherapy, which may impact long-term patient survival.
