Effect of comorbid benign joint hypermobility and juvenile fibromyalgia syndromes on pediatric functional gastrointestinal disorders

Nilufer Ulkue Sahin et al.

Summary: This study evaluated the association between benign joint hypermobility syndrome (BJHS) and juvenile fibromyalgia syndrome (JFMS) with functional gastrointestinal (GI) disorders. The findings showed that 64% of children with functional GI disorders had JFMS, and 32% had BJHS. JFMS patients commonly experienced retrosternal chest pain, dysphagia, early satiation, nausea, vomiting, and regurgitation. Gastrointestinal symptoms did not differ significantly in BJHS patients. These results suggest that emotional distress plays a role in the complex relationship between functional GI disorders, JFMS, and BJHS, and a multidisciplinary approach is necessary for diagnosis and treatment.

Celiac Disease in Children: A 2023 Update

Rishi Bolia et al.

Summary: Celiac disease is a chronic immune-mediated enteropathy caused by the ingestion of gluten in genetically predisposed individuals. It has a wide range of clinical manifestations and can affect people of all ages. The diagnosis is based on clinical history, serologic testing and duodenal biopsies. The management involves lifelong complete dietary avoidance of gluten, and monitoring of IgA-TGA for mucosal healing.

Assessment of Intestinal Permeability and Inflammation Bio-Markers in Patients with Rheumatoid Arthritis

Christina Heidt et al.

Summary: This study investigated the association between zonulin, calprotectin, and lipopolysaccharide (LPS) levels in RA patients. Serum zonulin levels were abnormal with longer disease duration, and fecal zonulin levels were inversely associated with age. There was a strong association between fecal and serum calprotectin, and between fecal calprotectin and LPS in males, suggesting the potential of fecal calprotectin as a specific biomarker for intestinal inflammation in RA.

Prevalence and detection rate of celiac disease in Italy: Results of a SIGENP multicenter screening in school-age children

Elena Lionetti et al.

Summary: The prevalence of celiac disease in school-age Italian children is one of the highest in the world, with 60% of patients currently undiagnosed without a mass screening strategy. Determination of HLA predisposing genotypes is an easy and fast first-level screening test for celiac disease.

Summary: This study provides new data on the prevalence of joint involvement in pediatric CD and its correlation with disease activity and quality of life. The results show that joint involvement is more prevalent in pediatric CD than previously reported and is associated with more severe disease activity and lower quality of life.

Celiac Disease in Juvenile Idiopathic Arthritis and Other Pediatric Rheumatic Disorders

Dimitri Poddighe et al.

Summary: This article reviews and analyzes clinical studies on celiac disease (CD) among children with juvenile idiopathic arthritis (JIA). The findings suggest a high prevalence of CD in JIA patients, particularly in those who are asymptomatic. However, there is no evidence supporting periodic CD screening for children with other pediatric rheumatic diseases.

Impact of delay in diagnosis in patients with celiac disease: A study of 570 patients at a tertiary care center

Jahnvi Dhar et al.

Summary: Delayed diagnosis of celiac disease has a significant impact on its systemic and laboratory manifestations. Patients with delayed diagnosis often present with atypical symptoms and experience a higher incidence of anemia, pubertal delay, and menstrual irregularities. They also show lower levels of hemoglobin, ferritin, transferrin saturation, and vitamin D. These findings emphasize the importance of early diagnosis and treatment.

Coeliac disease

Carlo Catassi et al.

Summary: Coeliac disease is an autoimmune disorder primarily affecting the small intestine and caused by the ingestion of gluten. It has a complex immune response mechanism. The disease has diverse clinical symptoms, including gastrointestinal symptoms, extraintestinal manifestations, and subclinical cases. Diagnosis involves serological testing and small-intestinal biopsy, and treatment includes a gluten-free diet.

Summary: This study found that fecal beta-defensin-2 and anti-BPI antibodies were significantly increased in children with active celiac disease. Fecal calprotectin levels were slightly increased. These biomarkers may have distinct functional roles in intestinal mucosal defense.

Risk of Juvenile Idiopathic Arthritis and Rheumatoid Arthritis in Patients With Celiac Disease: A Population-Based Cohort Study

Summary: This study shows that children with celiac disease (CD) have a nearly threefold increased risk of juvenile idiopathic arthritis (JIA) compared to the general population, while adults with CD have a twofold increased risk of rheumatoid arthritis (RA). Clinicians should be alert to the possibility of JIA or RA in patients with CD who present with joint symptoms.

Clinical Presentation in Children With Coeliac Disease in Central Europe

Petra Riznik et al.

Summary: The study found that abdominal pain has become the most common symptom in children with CD, with abdominal pain and asymptomatic presentation being more common in older children and adolescents, while symptoms of malabsorption are more common in younger children. This suggests a shift in clinical presentation of CD towards nonclassical presentation and asymptomatic disease.

Extraintestinal Manifestations of Inflammatory Bowel Disease: Current Concepts, Treatment, and Implications for Disease Management

Gerhard Rogler et al.

Summary: Inflammatory bowel diseases (IBDs) are systemic diseases that can affect not only the gut but also other organs, impacting the quality of life for patients. Understanding the prevalence, pathophysiology, and clinical presentation of extraintestinal manifestations (EIMs) is crucial for effective IBD treatment. EIMs commonly affect joints, skin, or eyes, but may also involve other organs.

Summary: Celiac Disease has been found to be as prevalent in certain Asian regions such as the Indian subcontinent and Middle East countries as in Western countries, but information about its epidemiology is lacking in other areas. In regions like Russia and Central Asia, the prevalence of CD may be underestimated due to various factors contributing to underdiagnosis.

Serum Zonulin Measured by Commercial Kit Fails to Correlate With Physiologic Measures of Altered Gut Permeability in First Degree Relatives of Crohn's Disease Patients

Namita Power et al.

Summary: In a cohort of 39 healthy relatives of Crohn's disease patients, there was no significant correlation found between serum ZO-1 levels and physiological measures of intestinal permeability such as LMR. This suggests that ZO-1 and I-FABP are not reliable markers of gut permeability as defined by LMR.

Summary: Increased intestinal permeability is associated with inflammatory bowel diseases, and fecal ZRP levels may serve as a noninvasive biomarker of disease activity. The study found a slight correlation between ZRP levels and disease activity in CD patients, but not in UC patients. Furthermore, fecal ZRP strongly correlated with FCP levels.

Summary: The study evaluated the reliability of a multiplex CytoBead CeliAK immunoassay in detecting anti-tTG IgA positive patients compared to a conventional single-parameter ELISA. Results showed a high concordance rate between the two methods, demonstrating substantial equivalency in assessing anti-tTG IgA antibody for CD screening in children. CytoBead CeliAK immunoassay may offer pre-analytical, analytical, and economic advantages.
