P53 expression in cytology samples may represent a marker of early-stage cancer

Federica Policardo et al.

Summary: The TP53 gene plays a crucial role in cell proliferation control and signaling regulation. TP53 mutation is associated with the malignant transformation of thyroid cells and tumor progression. This study found that the expression of p53 in cytology samples was correlated with malignant pathology diagnosis. p53 may be a useful marker for distinguishing thyroid follicular lesions.

Summary: Despite the reliance on morphological assessment, molecular testing can enhance the diagnostic and prognostic value of thyroid fine-needle aspiration cytology (FNAC). This study utilized ddPCR to detect TERT promoter mutations in preoperative FNAC material, and found that TERT mutations were associated with malignant disease and higher Ki-67 proliferation indices.

Different Threshold of Malignancy for RAS-like Thyroid Tumors Causes Significant Differences in Thyroid Nodule Practice

Summary: Histopathological diagnosis of papillary thyroid carcinomas (PTCs) is prone to significant observer variation due to different thresholds of RAS-like nuclear changes among pathologists. This variation in diagnosis of RAS-like thyroid tumors among practices underlies several issues in thyroid nodule practice.

The Italian Consensus for the Classification and Reporting of Thyroid Cytology: Cytohistologic and molecular correlations on 37,371 nodules from a single institution

Liborio Torregrossa et al.

Summary: The Italian Consensus for the Classification and Reporting of Thyroid Cytology (ICCRTC) includes six diagnostic categories, each indicating a different risk of malignancy. This monocentric retrospective study aimed to evaluate the distribution of ICCRTC classes and their cytohistologic correlations. The results showed that nondiagnostic and benign categories were the most common, but surprisingly had high malignancy rates. The suspicious and malignant categories had high specificity for malignancy, and the ICCRTC was reliable in identifying aggressive histotypes of thyroid carcinoma.

Summary: The p53 protein, known as the guardian of the genome, plays a critical role in preserving genomic integrity, and mutations in the TP53 gene can lead to uncontrolled cell proliferation and cancer formation. With p53 deficiency being common in human cancer, restoring p53 function may be an effective approach for cancer treatment.

Summary: Thyroid tumors derived from follicular cells are heterogeneous in nature, with variable morphological appearance and differentiation levels. Molecular markers play a significant role in both diagnostic and prognostic stratification, especially in the presurgical setting and for identifying genetic alterations related to prognosis. The advent of targeted drugs has further emphasized the importance of molecular characterization in guiding therapeutic strategies for patients with advanced tumors.

Significance of RAS Mutations in Thyroid Benign Nodules and Non-Medullary Thyroid Cancer

Vincenzo Marotta et al.

Summary: Only about 4% of thyroid nodules are carcinomas and require surgery. Fine-needle aspiration cytology is the most accurate tool to distinguish benign from malignant thyroid nodules, however about 30% of cases yield an indeterminate result. Testing for RAS mutations has limited utility due to its poor specificity and sensitivity.

Diagnostic and prognostic value of a 7-panel mutation testing in thyroid nodules with indeterminate cytology: the SWEETMAC study

Summary: The study evaluated the diagnostic and prognostic value of a 7-panel mutation testing in thyroid nodules with indeterminate cytology. It found that the mutation testing significantly increased the overall risk of cancer in mutated nodules and decreased it in wild-type nodules, confirming the relevance of this testing in diagnosing nodules with IC. However, genetic testing did not predict outcomes in the cancer patient subgroup.
