Evaluation of the Reveal® rapid AST system to assess the susceptibility of Pseudomonas aeruginosa from blood cultures

Summary: This study evaluated the performance of the Reveal (R) rapid AST system in determining the drug susceptibility of Pseudomonas aeruginosa strains directly from blood cultures. The Reveal (R) AST system provided MIC results for the 11 antipseudomonal antibiotics tested within a mean time to result of 6 h 22 min. Comparing to the GN6F panel, it showed a high overall rate of categorical agreement (96.1%) and low rates of major errors (4.2%) and minor errors (0.6%).

Nursing experiences in antimicrobial optimisation in the intensive care unit: A convergent analysis of a national survey

Summary: This study aimed to explore nurses' perceptions and experiences of antimicrobial stewardship in ICUs in Australia. The survey results showed that lack of education was a major barrier for nurses to engage in antimicrobial stewardship. Only a small proportion of nurses received education and training in antimicrobial stewardship. Educational gaps, noninclusive discussions, moral distress, and potential workload burden were identified as barriers to engagement.

Epidemiology and outcomes of hospital-acquired bloodstream infections in intensive care unit patients: the EUROBACT-2 international cohort study

Summary: This study investigated hospital-acquired bloodstream infections (HA-BSI) in 2600 adult patients from 333 ICUs in 52 countries. HA-BSI were frequently caused by Gram-negative, carbapenem-resistant, and difficult-to-treat pathogens. Antimicrobial resistance led to delays in adequate antimicrobial therapy, resulting in high mortality rates.

Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Exploiting genomics to mitigate the public health impact of antimicrobial resistance

Summary: Antimicrobial resistance (AMR) is a significant global public health threat caused by excessive use of antimicrobials. The interplay between pathogens, AMR encoding genes, and mobile genetic elements on a microbial level is not fully understood, hampering control measures. Whole-genome sequencing (WGS) and metagenomic analyses have improved our understanding of these processes, informing mitigation strategies and novel diagnostic methods to optimize antimicrobial use and control AMR. Future opportunities for AMR control are identified, alongside challenges that need to be addressed to fully utilize WGS approaches for international AMR control.

Summary: Next-generation sequencing technologies, including whole genome sequencing, targeted metagenomics sequencing, and shotgun metagenomics sequencing, are increasingly used in clinical microbiology diagnostics. These applications are used for initial identification of pathogens, detection of antimicrobial resistance mechanisms, and epidemiological tracking. They are also being utilized in public health, basic research, and clinical microbiology laboratory environments to identify and track foodborne outbreaks, nosocomial infections, and potential pathogens. Implementing these technologies requires consideration of important factors, and their potential in infectious disease diagnosis in the next 5 years is promising.

Summary: A study comparing VHA hospitals with and without on-site Infectious Disease (ID) specialists found that patients at hospitals with ID specialists received antibiotics more in line with stewardship principles, with lower total antibacterial exposure. The presence of an ID specialist was associated with lower total inpatient antibacterial use and more appropriate use of broad-spectrum and narrow-spectrum antibiotics.

Summary: The survey revealed that many respondents in intensive care units have antimicrobial stewardship programs and use therapeutic drug monitoring and optimized administration of antibiotics. The availability of an antimicrobial stewardship program is associated with a higher utilization of therapeutic drug monitoring. It is recommended for intensivists to actively seek antimicrobial susceptibility reports and to monitor antimicrobial use.
