The WHO AWaRe (Access, Watch, Reserve) antibiotic book and prevention of antimicrobial resistance

Summary: There is a lack of guidance on the appropriate use of antibiotics for common infections in many settings. The recently released WHO AWaRe antibiotic book provides specific guidance on the use of antibiotics in different settings and emphasizes the risk of antimicrobial resistance. The book covers 34 common infections and includes recommendations on the use of last-resort Reserve antibiotics, as well as the use of first-line Access antibiotics or no antibiotic care when it is the safest approach. It can help achieve the WHO target of increasing the proportion of global consumption of Access antibiotics to at least 60% and contribute to improving universal health coverage.

Impact of antimicrobial stewardship interventions to improve antibiotic prescribing for hospital inpatients in Africa: a systematic review and meta-analysis

Summary: This review examined the effectiveness of interventions to improve antibiotic prescribing for hospital inpatients in Africa. The findings suggest that multifaceted interventions increased compliance, reduced antibiotic utilization and cost, and had a slight or no difference in mortality and length of hospital stay. Meta-analysis results showed a decrease in relative risk of mortality and a decrease in length of hospital stay. Additionally, a decrease in resistance to most micro-organisms was observed.

Ongoing Efforts to Improve Antimicrobial Utilization in Hospitals among African Countries and Implications for the Future

Summary: There are increasing concerns with rising antimicrobial resistance (AMR) globally, and Africa has the highest rates of AMR. National action plans (NAPs) and initiatives, such as point prevalence surveys (PPS) and antimicrobial stewardship programs (ASPs), have been implemented across Africa to reduce AMR. The results show a high utilization of antimicrobials in hospitals, with penicillins, ceftriaxone, and metronidazole being the most prescribed antibiotics. ASPs have been initiated to improve future prescribing, and various suggested activities have been presented to stakeholders to reduce AMR in Africa.
