Effects of hydrocortisone combined with vitamin C and vitamin B1 versus hydrocortisone alone on microcirculation in septic shock patients: A pilot study

Summary: This study aims to investigate the effects of hydrocortisone combined with vitamin C and vitamin B1 on sublingual microcirculation in septic shock patients. Septic shock patients admitted to the ICU were randomly assigned to receive either hydrocortisone combined with vitamin C and vitamin B1 or hydrocortisone alone. The results showed that the treatment group had significantly higher perfused small vascular density (sPVD) at 4 hours and 24 hours after treatment compared to the control group.

Direct assessment of microcirculation in shock: a randomized-controlled multicenter study

Summary: This study aimed to determine whether considering sublingual microcirculatory perfusion variables improves the 30-day mortality in shock patients in the ICU. Results showed that although more patients in the intervention group had adjustments in medication or fluids within the next hour, there was no significant difference in microcirculatory values 24 hours later and 30-day mortality. Thus, integrating sublingual microcirculatory perfusion variables in the therapy plan did not improve patient survival.

Morphologic Mapping of the Sublingual Microcirculation in Healthy Volunteers

Summary: This study describes the microvascular anatomy of sublingual and oral microcirculation in a healthy population. The results show that the sublingual area has a homogenous distribution in terms of total/functional vessel density, focus depth, and small vessel mean diameters. This study provides a description of normal microvascular anatomy for future research on microcirculatory assessment.

Assessment of sublingual microcirculation for the screening of diabetic nephropathy

Summary: This study investigated the potential of employing sublingual microcirculation as an early noninvasive screening technique for diabetic nephropathy (DN). The results showed that microvascular density and perfusion in the sublingual microcirculation were negatively correlated with fasting plasma glucose and HbA1c. Furthermore, diabetes patients had reduced sublingual microcirculation compared to healthy individuals, and more severe reductions were found in those who developed microvascular complications. Sublingual microcirculation monitoring could be used as a noninvasive method for early detection of DN.

Summary: This study found that critically ill coronavirus disease 2019 patients exhibited sublingual microcirculatory alterations characterized by increased vessel density, capillary hematocrit, and RBC velocity, as well as higher numbers of leukocytes and RBC microaggregates in the microcirculation. The response of the microcirculation to coronavirus disease 2019-induced hypoxemia seems to be to increase its oxygen-extraction capacity by increasing RBC availability, while inflammation and hypercoagulation are apparent in the microcirculation.

Summary: Microvascular dysfunction in septic shock plays a crucial role in organ dysfunction and unfavorable outcomes, with alterations in microcirculation having profound implications and response to therapy being highly variable and unpredictable by traditional hemodynamic assessment tools. Understanding the processes involved in microvascular dysfunction is important for improving tissue perfusion beyond systemic hemodynamic variables.

Microcirculation alterations in severe COVID-19 pneumonia

Summary: This study evaluated the sublingual microcirculation of 27 mechanically ventilated patients with COVID-19 pneumonia and found altered tissue perfusion, characterized by high vascular densities and decreased proportion of perfused vessels. These findings suggest that COVID-19 may lead to microcirculatory abnormalities, potentially related to enhanced angiogenesis or hypoxia-induced capillary recruitment.

Summary: The human validation study of AVA 4.1 showed that it is not suitable for research or clinical purposes. There was poor agreement and no correlation between AVA 4.1 and AVA 3.2. Changes in microcirculatory states after induction of general anesthesia could be significantly measured with AVA 3.2, but not replicated with AVA 4.1.
