Effect of pharmacogenetic variations on praziquantel plasma concentration and safety outcomes among school children in Rwanda

Summary: School-based mass drug administration of Praziquantel (PZQ) is effective in eliminating schistosomiasis, but genetic variations in drug metabolizing enzymes can influence drug exposure and treatment outcomes. This study investigated the impact of pharmacogenetic variations on PZQ plasma concentrations and safety outcomes among Rwandan schoolchildren who received PZQ and albendazole. The genotypes of CYP2C9, CYP2C19, and CYP3A4 were significantly associated with PZQ concentration and its metabolite ratios. Lower cis-4-OH-PZQ/PZQ ratio was a predictor of adverse events following mass drug administration.

Efficacy and safety of praziquantel preventive chemotherapy in Schistosoma mansoni infected school children in Southern Ethiopia: A prospective cohort study

Summary: This study evaluated the effectiveness of single-dose praziquantel against Schistosoma mansoni infection and the safety of praziquantel plus albendazole preventive chemotherapy in school children. The results showed that single-dose praziquantel is effective for the treatment of intestinal schistosomiasis and the combination therapy is safe and tolerable. However, reduced effectiveness was observed in moderate-to-heavy infection and adverse events were reported in about one-fifth of infected children.

Prevalence and Determinants of Schistosoma mansoni Infection among Pre-School Age Children in Southern Ethiopia

Summary: The prevalence and factors associated with Schistosoma mansoni infection among pre-school-age children in Southern Ethiopia were assessed. The overall prevalence of S. mansoni infection was 14.3%, with significantly higher rates among 6-year-old and 7-year-old children. Accompanying others to water sources and residing near infested water sources increased the odds of S. mansoni infection. The study highlights the urgent need to include pre-SAC aged 4 to 7 years in preventive chemotherapy campaigns.

Effect of preventive chemotherapy with praziquantel on schistosomiasis among school-aged children in sub-Saharan Africa: a spatiotemporal modelling study

Summary: In sub-Saharan Africa, the prevalence of schistosomiasis among school-aged children decreased from 23.0% in 2000-10 to 9.6% in 2015-19, representing an overall reduction of 58.3%. The substantial decrease in prevalence is likely attributed to the scale-up of preventive chemotherapy interventions. It is necessary to maintain the progress in schistosomiasis control through various interventions, including preventive chemotherapy.

Summary: This study aimed to investigate the incidence and risk factors of adverse events (AEs) following mass administration of praziquantel and albendazole. The results showed that this treatment method is safe and well-tolerated, with the majority of AEs being mild or moderate and resolving within 3 days. There were significant differences in the incidence of AEs between different genders and age groups.

Summary: Preventive chemotherapy with praziquantel and albendazole co-administration is a global strategy to eliminate schistosomiasis and soil-transmitted helminth. This study conducted active safety surveillance to assess the adverse events following the mass drug administration of praziquantel and albendazole in school children. The results showed that the treatment was generally safe, but children infected with parasites were more likely to experience adverse events.

Summary: The study found a significant association between CYP2C19 genotypes and PZQ concentrations, with higher levels in CYP2C19 (*2, *3) carriers compared to ultra-rapid metabolizers (CYP2C19 *17 carriers). The metabolic ratio was also significantly different between CYP2C19 *17 carriers and CYP2C19 (*2, *3) carriers. No significant impact of CYP3A4, CYP3A5, CYP2C19, and CYP2C9 genotypes on treatment efficacy or adverse events was observed.
