Thioredoxin/Glutaredoxin Systems and Gut Microbiota in NAFLD: Interplay, Mechanism, and Therapeutical Potential

N-Acetylcysteine alleviates high fat diet-induced hepatic steatosis and liver injury via regulating the intestinal microecology in mice

Summary: N-Acetylcysteine (NAC), a well-accepted antioxidant, has been shown to protect against high-fat diet-induced non-alcoholic fatty liver disease (NAFLD) in mice by modulating the homeostasis of the gut microbiota. The study found that NAC supplementation improved obesity, dyslipidemia, and liver dysfunction caused by high-fat diet, and restored the disorder of the gut microbiota. The beneficial effects of NAC on NAFLD were dependent on the presence of intestinal microorganisms and involved the regulation of hepatic mRNA expression in relation to specific bacterial phyla and genera.

Summary: Non-alcoholic fatty liver disease (NAFLD) is the most common chronic liver disease worldwide, often starting as a benign liver fat accumulation that can progress to non-alcoholic steatohepatitis (NASH) and increase the risk of cirrhosis and hepatocellular carcinoma (HCC). Recent studies have highlighted the role of gut dysbiosis, which can lead to increased gut permeability and the release of toxic bacterial products and proinflammatory cytokines, causing and sustaining inflammation. The composition of the gut microbiota and various metabolites may vary during different stages of NAFLD and among population groups, and further research is needed to determine the causal relationship between gut microbiota profiles and NAFLD in patients. Additionally, multiple metabolites may contribute to the pathogenesis of NAFLD.

Summary: High dietary cholesterol leads to sequential progression of steatosis, steatohepatitis, fibrosis and eventually HCC in mice, concomitant with insulin resistance. Cholesterol-induced NAFLD-HCC formation is associated with gut microbiota dysbiosis, with specific bacterial composition changes between stages of liver disease. Alterations in gut bacterial metabolites were also observed with dietary cholesterol intake. Therapies targeting cholesterol and gut microbiota may be effective for preventing NAFLD-HCC.
