Summary: This study explores the pathogenesis of acute rheumatic fever (ARF) and its major manifestations, rheumatic valvular heart disease (RHD) and Sydenham chorea (SC). By examining IgG subclass reactions, it was found that IgG2 significantly interacts with GlcNAc in RHD and SC patients, distinguishing them from uncomplicated pharyngitis patients. Furthermore, GlcNAc-specific IgG2 was found in the cerebrospinal fluid of SC patients. In RHD patients, IgG2 preferentially deposited in heart tissues. These findings suggest that GlcNAc-specific IgG2 may play a role in the pathogenesis of group A streptococcal sequelae.

The Streptococcus pyogenes vaccine landscape

Summary: Efforts have been made in the development of Streptococcus pyogenes (Group A Streptococcus) vaccine, but hurdles in science, regulation, and commercialization remain, resulting in a limited vaccine pipeline. There is a global need to expedite the development process to address the significant burden caused by this pathogen. Building on a century of vaccine development, there are currently eight candidates in progress, with four based on M proteins and four on non-M protein antigens. These candidates have shown the potential to protect against S. pyogenes in preclinical models, with one candidate already demonstrating safety and immunogenicity in a Phase 1 trial, and several others preparing for clinical trials. To maintain progress, the Strep A Vaccine Global Consortium (SAVAC) was formed to accelerate global vaccine development. This article highlights the past, present, and future of S. pyogenes vaccine development, emphasizing key priorities and the role of SAVAC.
