Immune dysregulation

Andrew Long et al.

Summary: The understanding of immune dysregulation in various diseases continues to expand, with a focus on the role of altered microbiome, gut barrier dysfunction, and age-associated B cells in systemic inflammation. T follicular regulatory cells and their potential therapeutic interventions are also highlighted. Studies on juvenile idiopathic arthritis and hyperinflammatory syndromes provide insights into their pathogenesis and potential biomarkers and therapeutic targets for these conditions. Future trials are needed to address remaining questions regarding the pathogenesis, diagnosis, and treatment of autoimmune, inflammatory, and immunodeficiency syndromes.

Summary: TLR plays important roles in detecting pathogen-associated molecular patterns and shaping immune responses. This review discusses the host TLR response during HIV-1 infection and its co-infection with HBV or HCV, as well as the use of TLR agonists as a potential cure for HIV. This understanding will help develop new strategies for curing HIV-1 mono-infection or co-infection with HBV or HCV.

Managing the Risk of Infection in Chronic Lymphocytic Leukemia in the Era of New Therapies

Daniel Rivera et al.

Summary: Patients with CLL are prone to infections, and the use of novel therapies has increased in recent years. However, infections remain a significant complication, and there is limited evidence supporting infection prevention strategies. Timely prevention, recognition, and treatment of infections should be a key focus in the management of CLL patients receiving novel therapies.

Toll-Like Receptor Signaling and Its Role in Cell-Mediated Immunity

Tianhao Duan et al.

Summary: This article provides a detailed overview of the innate immune system's defense mechanisms against pathogens, with a focus on the role of Toll-like receptors (TLRs) in immune responses. TLRs not only link innate immunity to adaptive immunity, but also directly regulate the function of T cells. In addition, the article discusses the critical role of TLR signaling in host defense against infectious diseases, autoimmune diseases, and cancer.

Secondary Immune Deficiency and Primary Immune Deficiency Crossovers: Hematological Malignancies and Autoimmune Diseases

Mark Ballow et al.

Summary: Primary immunodeficiencies (PIDs) are genetic immune errors determined at birth, while secondary immunodeficiencies (SIDs) are acquired diseases with decreased immune cell counts. Distinguishing between primary and secondary antibody deficiencies is crucial in the era of immune-modulating biologicals. This article explores the crossovers between SIDs and PIDs, and discusses the challenges in diagnosis and treatment.

TLR9 expression in chronic lymphocytic leukemia identifies a promigratory subpopulation and novel therapeutic target

Emma Kennedy et al.

Summary: Despite the advancements in B-cell receptor-targeted inhibitors, CLL remains incurable. TLR9 activation by unmethylated DNA is associated with disease progression in CLL. High TLR9 expression promotes CLL cell migration and disease progression, while dual targeting of TLR9 and BTK shows strong synergism as a potential treatment strategy for this incurable disease.

T Cells in Chronic Lymphocytic Leukemia: A Two-Edged Sword

Elisavet Vlachonikola et al.

Summary: Chronic lymphocytic leukemia remains incurable despite recent progress in management, highlighting the importance of further investigating the underlying pathophysiology. The interactions between malignant CLL cells and the tumor microenvironment play a crucial role in disease progression, with T cell abnormalities and potential therapeutic targets being a key focus in enhancing targeted cytotoxic activity against the malignant clone.

Secondary immunodeficiencies An overview

Karen S. Tuano et al.

Summary: Secondary immunodeficiencies have various causes, including malnutrition, metabolic disorders, medication use, chronic infections, malignancies, and more. Immune responses may be weaker in neonates and the elderly. Clinicians should consider secondary immunodeficiencies in patients with recurrent infections and abnormal immunologic evaluation.
