The Impact of Oversizing in Thoracic Endovascular Aortic Repair on Long-Term Outcomes in Uncomplicated Type B Aortic Dissection: A Single-Center Retrospective Study

Dongqiao Xiang et al.

Summary: The impact of oversizing in thoracic endovascular aortic repair (TEVAR) on survival and adverse events in patients with uncomplicated type B aortic dissection (TBAD) was assessed. Results showed that oversizing >5% was associated with an increased risk of retrograde type A aortic dissection within a year of TEVAR.

Effectiveness of Proximal Landing Zone 1 and 2 Thoracic Endovascular Aortic Repair for Type B Aortic Dissection by Comparing Outcomes With Thoracic Arch Aneurysm

Tomoaki Kudo et al.

Summary: This study aimed to clarify the effectiveness and expand the possibilities of zone 1 and 2 landing TEVAR for type B aortic dissection (TBAD) using our treatment strategy. Satisfactory early and long-term results were obtained with zone 1 and 2 landing TEVAR, and the TBAD cases had the same good outcomes as the thoracic arch aneurysm (TAA) cases. Our treatment strategy may reduce complications and be an effective option for acute complicated TBAD.

Influence of measurement and sizing techniques in thoracic endovascular aortic repair on outcome in acute complicated type B aortic dissections

Miriam Rychla et al.

Summary: In patients with cTBAD, TEVAR oversizing by <= 10% may reduce aortic-related events by up to 50%. Different measurement techniques for pre-dissection aortic diameter showed variances of 1.7-4.0 mm, with a maximum variability of 8.4 mm.

A more proximal landing zone is preferred for thoracic endovascular repair of acute type B aortic dissections

Tomaz Mesar et al.

Summary: Most patients with aTBAD have less than 2 cm of healthy descending thoracic aorta. In patients treated for complicated aTBAD, Z2T is associated with a lower need for aortic reintervention and aortic-related adverse events than Z3T. Patients may benefit from a more aggressive proximal landing zone with similar perioperative morbidity when Z2T is done with LSA revascularization.

Long-term results of total endovascular repair of arch-involving aortic pathologies using parallel grafts for supra-aortic debranching

Philip Dueppers et al.

Summary: This study aimed to evaluate the long-term outcomes of thoracic endovascular aortic repair combined with parallel grafts (PG-TEVAR) for arch-involving aortic pathologies. The results showed a high rate of type I endoleaks and the need for long-term reintervention in patients undergoing PG-TEVAR. Further studies comparing PG-TEVAR to other total endovascular alternatives are needed to confirm these findings.

Early and Mid-Term Outcomes of Open versus Endovascular Left Subclavian Artery Debranching for Thoracic Aortic Diseases

Philip Dueppers et al.

Summary: This study compared the outcomes of open versus endovascular left subclavian artery debranching for thoracic endovascular aortic repair and found that open debranching had higher technical and clinical success rates and fewer type I endoleaks.

Current options and recommendations for the use of thoracic endovascular aortic repair in acute and chronic thoracic aortic disease: an expert consensus document of the European Society for Cardiology (ESC) Working Group of Cardiovascular Surgery, the ESC Working Group on Aorta and Peripheral Vascular Diseases, the European Association of Percutaneous Cardiovascular Interventions (EAPCI) of the ESC and the European Association for Cardio-Thoracic Surgery (EACTS)

Martin Czerny et al.

Summary: TEVAR has become the standard treatment for thoracic aortic diseases, but the importance of open surgery as a complementary option is also emphasized in cases of late failures. Formation of aortic centers and centralization of care are crucial for continuing development and improving outcomes. The future challenge lies in addressing complex issues such as the aortic root.

Determination of Optimal and Safest Proximal Sealing Length During Thoracic Endovascular Aortic Repair

Michele Piazza et al.

Summary: The study aimed to determine the optimal and safest proximal sealing length during thoracic endovascular aortic repair (TEVAR). For type II and III aortic arches, a proximal sealing length of 25-30mm may be required for a safe and durable procedure.
