Summary: Intrapartum hypoxia-ischemia leading to neonatal encephalopathy is a major cause of neonatal mortality and morbidity worldwide, particularly in low- and middle-income countries. Therapeutic hypothermia is currently the only effective treatment available in high-income countries, but its effectiveness is limited in LMIC. In this study, we compared the effects of 25 potential neuroprotective drug candidates on neonatal hypoxic-ischemic brain injury in rats. We found that caffeine, Sonic Hedgehog Agonist (SAG), and Allopurinol had the strongest treatment effects, followed by Melatonin, Clemastine, ss-Hydroxybutyrate, Omegaven, and Iodide. These alternative therapies may hold promise for the treatment of hypoxic-ischemic brain injury in LMIC.

A phase I clinical trial was conducted to test the safety and efficacy of hCT-MSC in neonates with HIE, with positive results.

The results showed that erythropoietin treatment did not reduce neuroinflammation biomarkers or brain injury in infants with HIE, while circulating biomarkers moderately improved the estimation of 2-year outcomes.

Citicoline in hypoxic ischemic encephalopathy in neonates: a randomized controlled trial

Summary: This study evaluated the effect of citicoline as a neuroprotective drug in neonates with moderate and severe hypoxic-ischemic encephalopathy (HIE). The results showed a significant reduction in the number of neonates having seizures, improved cranial ultrasound and MRI findings, and better neurodevelopmental outcomes at 9 and 12 months in the citicoline treated group compared to the control group. There was also a statistically significant reduction in the duration of seizures, NICU stay, inotrope use, and mechanical ventilation in the treatment group.
