Automated Collateral Scoring on CT Angiography of Patients with Acute Ischemic Stroke Using Hybrid CNN and Transformer Network

Hulin Kuang et al.

Summary: This study proposes a vessel-segmentation free method for automating collateral scoring on CT angiography (CTA) to assist in the diagnosis and treatment decisions of acute ischemic stroke (AIS). The method utilizes maximum intensity projection and middle cerebral artery region segmentation to process the CTA images, and applies a hybrid CNN and Transformer model (MPViT) to determine the collateral scores automatically. Evaluation on 154 CTA scans of AIS patients demonstrates that the proposed method achieves high performance in both three-point collateral score classification and dichotomized classification.

Collateral Capacity Assessment Robustness and Interobserver Agreement of Two Grading Scales and Agreement with Quantitative Scoring

Wenjin Yang et al.

Summary: In order to address the coarseness and low interobserver agreement of the conventional Tan collateral score, we have introduced and evaluated an extended Tan score in this study. The results show that the extended score has good interobserver agreement and is consistent with quantitative assessment, indicating its potential to improve the prognostic value of collateral assessment in clinical practice.

Collateral Circulation in Ischemic Stroke: An Updated Review

Gino Maguida et al.

Summary: The collateral circulation is important in ischemic stroke as it maintains perfusion to brain tissue and allows for a longer time window for effective therapies. However, enhancing it as a therapeutic target remains a challenge. Assessment of the collateral circulation is now routine in neuroimaging for stroke, providing a more complete picture of the pathophysiology and aiding in treatment selection and outcome prediction. This review aims to provide an updated approach to the collateral circulation and highlight ongoing research with potential clinical applications.

Determinants of Leptomeningeal Collateral Status Variability in Ischemic Stroke Patients

Summary: This study found that male sex and leukocytosis are associated with poor collaterals in ischemic stroke patients. The measured variables only explained about half of the observed between-patient variance in collaterals, suggesting that genetic differences may play a role in collateral flow variability.

Summary: Digital Subtraction Angiography (DSA) is the gold-standard imaging modality for acute cerebrovascular diagnosis, with an increasingly prominent role in endovascular therapy for acute ischemic stroke. While primarily used for assessing cerebral vessel patency, further investigation is needed into the therapeutic role of DSA from a prognostic standpoint.

Favourable arterial, tissue-level and venous collaterals correlate with early neurological improvement after successful thrombectomy treatment of acute ischaemic stroke

Tobias Djamsched Faizy et al.

Summary: This study found that favorable pial arterial collaterals (PAC) and venous outflow (VO) were associated with early neurological improvement (ENI) after thrombectomy in patients with acute ischemic stroke due to large vessel occlusion (AIS-LVO). Additionally, favorable tissue-level collaterals (TLC) predicted longer term functional recovery after thrombectomy, but the impact of TLC on ENI strongly depended on vessel reperfusion.

Time Since Stroke Onset, Quantitative Collateral Score, and Functional Outcome After Endovascular Treatment for Acute Ischemic Stroke

Simone M. Uniken Venema et al.

Summary: In patients with ischemic stroke, the time to CT angiography (CTA) and collateral status are important factors for prognosis. This study aimed to assess the relationship between time to CTA and a quantitatively determined collateral score and whether the collateral score modified the relationship between time to recanalization and functional outcome. The results showed that time to CTA did not affect the collateral status, and a shorter time to recanalization was associated with better functional outcome regardless of the baseline collateral status.

Summary: Regularly updated data on stroke including incidence, prevalence, mortality, disability, risk factors, and trends are crucial for evidence-based care planning. The Global Burden of Diseases Study provides comprehensive measurement of these metrics. From 1990 to 2019, while age-standardized rates improved, the number of strokes and deaths increased. Low-income countries had the highest stroke-related mortality and DALY rates, with high body-mass index being the fastest-growing risk factor.
