The clinical significance of duration of untreated psychosis: an umbrella review and random-effects meta-analysis

Oliver D. Howes et al.

Summary: The concept that a longer duration of untreated psychosis (DUP) leads to poorer outcomes has prompted significant changes in mental health services and research interest worldwide over the past 30 years. An umbrella review of meta-analyses and a random-effects meta-analysis of primary studies were conducted to evaluate this notion. Strong associations were found between longer DUP and more severe negative symptoms, higher risk of previous self-harm, more severe positive symptoms, lower chance of remission, poorer overall functioning, and more severe global psychopathology. These findings suggest that DUP is an important prognostic factor in mental illness.

Establishing a clinical service to prevent psychosis: What, how and when? Systematic review

Gonzalo Salazar de Pablo et al.

Summary: To successfully translate prevention of psychosis into clinical practice, specialised Clinical High-Risk for Psychosis (CHR-P) services need to be established. This study reviewed 56 studies from 1998 to 2020 across 15 countries, finding that most services are provided by integrated multidisciplinary teams with active outreach strategies and referrals from healthcare agencies. CHR-P individuals are often male, aged 12-35, with services typically offering up to 2 years of clinical monitoring and various interventions.

F. Lejuste et al.

Summary: A study conducted at Sainte-Anne hospital in Paris found that the 16-item Prodromal Questionnaire (fPQ16) has good predictive value for diagnosing ultra-high-risk states (UHR) in psychosis, as well as good acceptability. The research results showed that the best cut-off score for fPQ16 was 4, with excellent sensitivity and correct specificity.

Probability of Transition to Psychosis in Individuals at Clinical High Risk An Updated Meta-analysis

Gonzalo Salazar de Pablo et al.

Summary: This study quantitatively analyzed the risk of transitioning from CHR-P individuals to psychosis, finding that the transition risk continues to increase in both short and long terms. Lower proportion of female individuals and higher frequency of brief psychotic symptoms were associated with increased transition risk.
