Effect of Catheter Ablation Using Pulmonary Vein Isolation With vs Without Posterior Left Atrial Wall Isolation on Atrial Arrhythmia Recurrence in Patients With Persistent Atrial Fibrillation: The CAPLA Randomized Clinical Trial

Peter M. Kistler et al.

Summary: This randomized clinical trial compares pulmonary vein isolation (PVI) vs PVI with posterior wall isolation for achieving freedom from atrial arrhythmia in adult patients with persistent atrial fibrillation undergoing first-time catheter ablation in Australia, the UK, and Canada. The study findings suggest that adding posterior wall isolation to PVI does not significantly improve freedom from atrial arrhythmias in patients with persistent AF. Therefore, the empirical inclusion of posterior wall isolation for ablation of persistent AF may not be supported.

Extensive Posterior Wall Isolation on Top of Pulmonary Vein Isolation Guided by Ablation Index in Persistent Atrial Fibrillation Ablation

Francesco Sabatino et al.

Summary: The study evaluated the efficacy of posterior wall isolation (PWI) guided by Ablation Index (AI) in addition to pulmonary vein isolation (PVI) for patients with persistent atrial fibrillation (Pe-AF). The results showed that AI-guided PWI on top of PVI reduced arrhythmia recurrence effectively.

Summary: The study investigates the use of high-power short-duration ablation for treating atrial fibrillation using a lesion size index-guided approach. The results demonstrate that this method is safe and effective, with no adverse events reported after a 1-year follow-up.

Summary: The study found that posterior wall isolation significantly reduced atrial fibrillation recurrence in patients with persistent AF compared to conventional methods. Subgroup analysis showed that PWI was effective in reducing AF recurrence, highlighting the benefit of using PWI in patients with persistent AF.

Summary: This study found that adding POBI to CPVI did not improve rhythm outcomes or affect overall safety in patients undergoing redo AF ablation with reconnected PVs, but it led to a longer ablation time compared to CPVI alone.

Positive Clinical Benefit on Patient Care, Quality of Life, and Symptoms After Contact Force-Guided Radiofrequency Ablation in Persistent Atrial Fibrillation Analyses From the PRECEPT Prospective Multicenter Study

Andrea Natale et al.

Summary: Contact force-guided radiofrequency ablation of persistent atrial fibrillation led to reduced cardioversion rate, decreased use of antiarrhythmic drugs, and improved quality of life in patients. Majority of patients were asymptomatic at 15 months post-ablation, indicating positive clinical impact on shared decision-making in treatment.

Summary: This study evaluated the impact of posterior wall isolation (PWI) on recurrent arrhythmia after catheter ablation for persistent atrial fibrillation (AF). The results showed a significantly lower rate of recurrent persistent AF in the PVI plus PWI group compared to the PVI only group.
