Comparison of Omental Patch Closure Versus Simple Closure for Laparoscopic Repair of Perforated Peptic Ulcer: A Systematic Review and Meta-Analysis

The impact of drug concentration on the genetic and phenotypic changes during fungal adaptation is not well understood. This study shows that different concentrations of the antifungal drug fluconazole can lead to distinct genotypic and phenotypic outcomes, which may contribute to the observed variation in drug response.

Laparoscopic Graham Patch for Anterior Duodenal Perforation in a 6-Year-Old

Summary: Perforated peptic ulcer in children is rare and usually occurs in teenagers. We report a case of a 6-year-old boy with abdominal pain and vomiting, who was found to have peptic ulcer perforation with CT findings of pneumoperitoneum and pelvic free fluid without a clear cause. Laparoscopic examination revealed an anterior duodenal ulcer and laparoscopic Graham patch repair was performed. The patient tested positive for H. pylori and received triple therapy for eradication.

Experience in diagnosis and treatment of duodenal ulcer perforation in children

Summary: This study retrospectively collected 52 cases of pediatric duodenal ulcer perforation in Beijing Children's Hospital, and divided them into surgery group and conservative treatment group. It was found that duodenal ulcer perforation is more common in adolescents, and Helicobacter pylori infection is the main cause. Conservative treatment is safe and feasible, but requires longer fasting time. Simple suturing is the main management for the surgery group.

However, more prospective studies are needed to evaluate the learning curve in LOPR.

Evolution of pediatric gastrointestinal ulcer disease: Is acute surgical intervention relevant?

Summary: A lack of contemporary data about pediatric gastrointestinal ulcer disease was addressed by a retrospective study from 1990 to 2019, which found that immunosuppressed children were more likely to require surgical intervention for ulcers. The study also showed that immunosuppressed patients have more complications, longer hospital stays, and are more likely to need surgical intervention compared to immunocompetent patients. Immunosuppressed pediatric patients with ulcers may benefit from ulcer prophylaxis and early investigation of epigastric pain.
