Nuclear Receptors Linking Metabolism, Inflammation, and Fibrosis in Nonalcoholic Fatty Liver Disease

Summary: Nonalcoholic fatty liver disease (NAFLD) and nonalcoholic steatohepatitis (NASH) are chronic liver diseases that can lead to end-stage liver disease and hepatocellular carcinoma (HCC) worldwide. The metabolic syndrome is closely linked to NAFLD, and comorbidities such as type 2 diabetes, obesity, and insulin resistance worsen liver disease, while NAFLD increases cardiovascular risk in affected patients. Therapeutic strategies targeting multiple drug targets or drugs with multiple mechanisms of actions are needed for effective treatment.

Cinnabarinic Acid Provides Hepatoprotection Against Nonalcoholic Fatty Liver Disease

Summary: In this study, it was demonstrated that cinnabarinic acid (CA), an endogenous AhR agonist, provides protection against non-alcoholic fatty liver disease (NAFLD). CA protects against steatosis and liver injury by controlling the expression of several key genes associated with lipid metabolism pathways, limiting hepatic lipid uptake, and controlling liver inflammation. Moreover, CA-induced hepatoprotection is absolutely dependent on AhR and Stc2 expression.

Nonalcoholic steatohepatitis (NASH) is characterized by liver steatosis, inflammation, hepatocyte damage, and fibrosis. Genetic predisposition and environmental factors such as dietary habits and obesity may contribute to the progression of NASH. The role of lipid-sensing nuclear receptors in the pathogenesis and treatment of NASH, including peroxisome proliferator-activated receptors, farnesoid X receptor, and liver X receptors, is discussed in this study.

Summary: The aryl hydrocarbon receptor (AHR) is considered a key regulatory factor in the immune system, playing an important role in integrating environmental, dietary, or microbial signals into adaptive immune responses in barrier organs such as the gut, skin, and lung. Despite being prominently exposed to AHR ligands, the role of AHR in regulating hepatic immune responses, which are biased towards tolerance, is still poorly understood. Further research is warranted to explore the potential therapeutic implications of targeting AHR in liver disorders such as autoimmune liver disease, liver fibrosis, and liver cancer.
