Clinical Significance of the Duality of Wnt/β-Catenin Signaling in Human Hepatocellular Carcinoma

Summary: Wnt/beta-catenin mutations in hepatocellular carcinomas (HCCs) can result in two distinct tumor phenotypes. The "Jekyll" phenotype, characterized by high HNF4 alpha status, is associated with low AFP levels, good prognosis, and repression of EMT. The "Hyde" phenotype, characterized by high FOXM1 status, is associated with high AFP levels, poor prognosis, and promotion of EMT. Imaging techniques such as Gd-EOB-DTPA-enhanced MRI and FGD-PET/CT can be used to detect the expression of OATP1B3 and GLUT1, respectively, which are markers for these phenotypes. Future research should focus on non-invasive detection of Wnt/beta-catenin activation and its application in the treatment of advanced HCC.

Summary: This study evaluated the use of liver-biliary phase of EOB-MRI to predict the therapeutic effect of lenvatinib and atezolizumab plus bevacizumab. The results showed that in the lenvatinib group, the liver-biliary phase did not stratify progression-free survival (PFS) differently, while in the atezolizumab plus bevacizumab group, patients with heterogeneous type and hyperintensity type had significantly shorter PFS.

Immunological Microenvironment Predicts the Survival of the Patients with Hepatocellular Carcinoma Treated with Anti-PD-1 Antibody

Summary: This study identified prognostic factors for anti-PD-1 antibody treatment in advanced HCC based on molecular and immunological features. Factors associated with better response included low alpha-fetoprotein levels, negative beta-catenin/GS staining, high CPS of PD-L1, and increased CD8(+) cell infiltration. Negative staining of beta-catenin/GS, CPS of PD-L1 >= 1, and high CD8(+) TILs were significantly associated with longer survival. These findings suggest that the combined score of Wnt/beta-catenin activation, CPS of PD-L1, and CD8(+) TILs can predict the response to ICI in HCC cases.

Summary: A prognostic grading system was proposed for patients undergoing resection of hepatocellular carcinoma, which divided patients into four surgical grades based on prognostic factors. The grades showed good reproducibility for predicting both overall and recurrence-free survival.
