Summary: The aim of this study was to evaluate the role of k index in the initial assessment of Multiple Sclerosis (MS) patients and to determine useful cut-off values for clinical practice. The results showed that kappa index is a quantitative biomarker for diagnosing and predicting prognosis of MS, as it is significantly associated with lesion load and clinical course. A cut-off value of k index >= 106 was proposed to identify patients at major risk of relapse, which can potentially impact initial therapeutic decisions.
