An analysis of existing national action plans for antimicrobial resistance-gaps and opportunities in strategies optimising antibiotic use in human populations

Esmita Charani et al.

Summary: At the 2015 World Health Assembly, UN member states committed to developing national action plans (NAPs) for antimicrobial resistance. The political determination and robust governance structures remain major barriers to progress. This study analyzed existing NAPs and found gaps and variability in different domains related to AMR, providing opportunities to improve and implement NAPs effectively.

Cultures of antibiotic prescribing in medical intensive care

Katharina Rynkiewich et al.

Summary: This study examines the culture of antibiotic prescribing in medical intensive care units in the United States, and finds that antibiotic use in these units is shaped by urgency, hierarchy, and uncertainty. The results indicate that by studying the culture of antibiotic prescribing in medical intensive care units, we can gain a clearer understanding of the vulnerability of the antimicrobial resistance crisis, as well as the perceived insignificance of stewarding antibiotic use compared to acute medical concerns.

Interventions to support the re-establishment of breastfeeding and their application in humanitarian settings: A systematic review

Nieves Amat Camacho et al.

Summary: This study aimed to compile and evaluate the characteristics, outcomes, and factors influencing the implementation of relactation support interventions. The findings suggest that relactation support can contribute to re-establishing breastfeeding, but its effectiveness in humanitarian settings remains uncertain.

A systems thinking approach for antimicrobial stewardship in primary care

Sajal K. Saha et al.

Summary: Although key AMS strategies are known, effective implementation of AMS programs in primary care is hindered by unclear health system components. Systems thinking is crucial in designing and implementing AMS programs, with a systemigram potentially being a useful tool to facilitate implementation.

Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Christopher J. L. Murray et al.

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Community engagement: The key to tackling Antimicrobial Resistance (AMR) across a One Health context?

Jessica Mitchell et al.

Summary: Antimicrobial resistance (AMR) is a complex issue driven by various factors, particularly prominent in low- and middle-income countries. Community Engagement (CE) has shown potential in facilitating behavior change towards AMR in these countries. However, there is a need for broader attention to be paid to AMR as a One Health problem, and a recognition of the challenges in evaluating and scaling CE interventions.

Antibiotic resistance among bacteria isolated from war-wounded patients at the Weapon Traumatology Training Center of the International Committee of the Red Cross from 2016 to 2019: a secondary analysis of WHONET surveillance data

Sally Yaacoub et al.

Summary: This study contributes to the understanding of antibiotic resistance burden among war-related wounds patients in the Middle East, revealing a high proportion of multi-drug resistant bacteria and identifying higher odds of MDR isolates in Iraqi patients and in Enterobacterales isolates. Regular screening and antibiotic-sensitivity based treatment are emphasized based on the findings.

Ethical guidance or epistemological injustice? The quality and usefulness of ethical guidance for humanitarian workers and agencies

Julian Sheather et al.

Summary: This paper examines the quality and usefulness of ethical guidance for humanitarian aid workers and their organizations, particularly in the context of public health emergencies. The study reveals a disconnect between ethical guidance and the actual experiences of frontline workers, and highlights the potential gap between those who produce ethical guidelines and those who face real-time challenges on the ground. It suggests the existence of epistemic control and the imposition of theoretical preoccupations on frontline humanitarian practice.

Torsten Schmitz et al.

Summary: The implementation of ALMANACH, a digital CDSS based on the Integrated Management of Childhood Illness in primary healthcare facilities in Adamawa State, Nigeria, significantly improved caregiver-reported recovery, reduced antimicrobial prescription rates, and increased rates of referral, communication of diagnosis, and follow-up advice for sick children.

Chronic NCD care in crises: A qualitative study of global experts' perspectives on models of care for hypertension and diabetes in humanitarian settings

Eimhin Ansbro et al.

Summary: In humanitarian crises, people with NCDs are particularly vulnerable. Current HTN/DM care models are mainly based at the primary-care level and show gaps in standardization, continuity of care, and coordination with other actors. Experts recommend strengthening health systems, providing patient-centred care, and conducting further research.

Delivering health interventions to women, children, and adolescents in conflict settings: what have we learned from ten country case studies?

Neha S. Singh et al.

Summary: The study examines the provision of health interventions in conflict-affected settings across ten countries, revealing prioritisation of antenatal care, emergency obstetric care, immunisation, and treatment of childhood illnesses despite variations in contexts. International donors play a significant role in influencing the implementation of women's and children's health services in conflict settings, with gaps in the delivery of reproductive, newborn, and adolescent health services.

Task-sharing interventions for improving control of diabetes in low-income and middle-income countries: a systematic review and meta-analysis

Joseph Linju Maria et al.

Summary: This study conducted a systematic review and meta-analysis of task-sharing intervention strategies for managing type 2 diabetes in low-income and middle-income countries. The results indicate that interventions led by nurses and pharmacists were effective in reducing HbA(1c) levels, while interventions by dietitians and community health workers showed weaker effects. For fasting blood sugar (FBS) levels, interventions led by pharmacists were more effective compared to nurses and community health workers.

Challenges and opportunities for infection prevention and control in hospitals in conflict-affected settings: a qualitative study

Hattie Lowe et al.

Summary: In conflict-affected settings, barriers to infection prevention and control (IPC) in hospitals include inadequate infrastructure, resource shortages, lack of education, and high visitor numbers. Conflict directly leads to increased patient numbers, supply chain disruptions, attacks on healthcare infrastructures, exacerbating existing challenges in IPC. Hospitals in these settings have adopted local strategies to improve IPC in the face of limited resources.

The socioeconomic burden of antibiotic resistance in conflict-affected settings and refugee hosting countries: a systematic scoping review

Elsa Kobeissi et al.

Summary: This systematic scoping review aimed to understand the socioeconomic burden of antibiotic resistance (ABR) in conflict-affected settings and refugee hosting countries. The review found that existing studies were mostly small and lacked sufficient information to accurately evaluate the cost incurred by antibiotic resistant infections. Further research is needed to comprehend the cost of ABR in these contexts and to develop adaptable policies.
