Correlation between extended pelvic lymph node dissection and urinary incontinence at early phase after robot-assisted radical prostatectomy

Ken Lee et al.

Summary: This study investigated the impact of extended pelvic lymph node dissection (ePLND) on urinary incontinence (UI) after robot-assisted radical prostatectomy (RARP). The results showed that the urinary continence rate at 1 month postoperatively was lower among patients who underwent ePLND, but there was no significant difference at 3, 6, and 12 months after RARP. Additionally, ePLND and larger prostate volume were associated with an increased UI rate.

Fifteen-Year Outcomes after Monitoring, Surgery, or Radiotherapy for Prostate Cancer

F. C. C. Hamdy et al.

Summary: A study in the UK showed that after 15 years of follow-up, the mortality rate of prostate cancer was low regardless of the treatment assigned. Therefore, the choice of therapy for localized prostate cancer involves considering the trade-offs between benefits and harms.

Patient-reported functional outcome measures and treatment choice for prostate cancer

Tenaw Tiruye et al.

Summary: This study compared the differences in patient-reported functional outcomes among common treatment approaches for prostate cancer. The study found that while urinary and bowel functions varied little across treatment groups, there were significant differences in sexual function. After adjustment, radical prostatectomy was associated with a greater decline in urinary continence and sexual function compared to other treatments.

Comparison of the Oncological and Functional Outcomes of Brachytherapy and Radical Prostatectomy for Localized Prostate Cancer

Fei Wang et al.

Summary: This study compared the oncological and functional outcomes of brachytherapy (BT) and radical prostatectomy (RP) in patients with localized prostate cancer. The results showed that BT and RP had comparable effects in terms of cancer control. Gleason score was the main independent prognostic factor for biochemical relapse-free survival. BT had better health-related quality of life (HRQoL) compared to RP in some aspects but returned to baseline after 2 years.

The Effect of Pelvic Floor Rehabilitation on Males with Sexual Dysfunction: A Narrative Review

David Yaacov et al.

Summary: Pelvic floor physical therapy treatment is found to have a positive impact on male premature ejaculation and erectile dysfunction. By providing education, rehabilitation, and manual therapy techniques, the pelvic floor muscles can be strengthened, leading to improvements in ED and PE. Most studies suggest that a multifaceted approach, combining manual physical therapy treatments with pelvic floor muscle training, is effective in managing ED and PE.

Biofeedback for Pelvic Floor Disorders

Melissa Hite et al.

Summary: Defecatory disorders, including fecal incontinence and functional anorectal pain, greatly impact quality of life and healthcare costs. Treatments for pelvic floor disorders, such as medications, surgery, physical therapy, and biofeedback, aim to enhance muscle strength and coordination. Biofeedback has shown effectiveness in treating chronic constipation, fecal incontinence, and low anterior resection syndrome, with studies suggesting it is superior to pelvic floor muscle training alone.

Comprehensive approach for preserving cavernous nerves and erectile function after radical prostatectomy in the era of robotic surgery

Nobuhiro Haga et al.

Summary: Nerve-sparing procedures are crucial for preserving postoperative erectile and lower urinary tract functions, presenting challenges due to the invisibility of cavernous nerves during robot-assisted radical prostatectomy. Continued development of new techniques and devices for preserving cavernous nerves is needed to improve surgical outcomes.

New Evolution of Robotic Radical Prostatectomy: A Single Center Experience with PERUSIA Technique

Giovanni Cochetti et al.

Summary: The study retrospectively analyzed data from 454 robotic-assisted radical prostatectomies using the PERUSIA technique, evaluating peri-operative, oncologic, and functional outcomes. The overall complication rate was 16%, with positive surgical margins at 8.1% and biochemical recurrence at 8.6%. Urinary continence was achieved in 69% of cases the day after catheter removal, increasing to 97% at 12 months post-surgery, while sexual potency rates were 72% and 82% at 3 and 12 months respectively.

Effect of Kegel exercises on the prevention of urinary and fecal incontinence in patients with prostate cancer undergoing radiotherapy

A. E. Urvaylioglu et al.

Summary: This nonrandomized experimental study aimed to evaluate the effect of Kegel exercises on the prevention of urinary and fecal incontinence in prostate cancer patients undergoing radiotherapy. The results indicate that Kegel exercises can significantly prevent urinary incontinence and improve quality of life. The intervention group showed lower levels of urinary incontinence and overall health improvement compared to the control group.

Summary: A relatively high prevalence of postoperative detrusor underactivity was observed in patients at 1 month after robot-assisted radical prostatectomy. Patients with preoperative low bladder contractile index have a higher probability of developing early postoperative detrusor underactivity after robot-assisted radical prostatectomy.

Summary: Among postmenopausal women, higher physical activity was associated with lower risk of incident urgency and mixed UI, but not stress UI, independent of baseline weight and weight change. Higher protein intake was associated with increased risk of urgency UI, but no associations were observed between other macronutrient and UI subtypes.

Summary: Urinary incontinence is a common complication post-radical prostatectomy and traditional postoperative interventions may not be as effective.
