Robotic surgery in emergency setting: 2021 WSES position paper

Nicola de'Angelis et al.

Summary: Robotics in emergency general surgery is a growing field with potential benefits, but requires careful patient selection and consideration of challenges such as availability, longer operative times, and increased costs. Further research is needed to explore telementoring and telesurgery in emergency settings, and hospitals should efficiently implement robotic surgical platforms while continuously reevaluating evidence.

Blunt thoracic trauma: role of chest radiography and comparison with CT - findings and literature review

Karunesh Polireddy et al.

Summary: This article emphasizes the importance of using portable chest radiography as the initial diagnostic test in acute trauma to identify various injuries resulting from blunt chest trauma. It compares the efficacy of chest radiography with computed tomography (CT) and discusses common radiographic findings in these injuries. The article highlights the role of chest radiography in timely diagnoses and prompt management in the setting of acute blunt chest trauma.

Never to be missed again - an analysis of 55 consecutive cases of traumatic diaphragmatic hernia

V. F. Kruger et al.

Summary: This study reviewed the clinical experience of 55 cases of traumatic diaphragmatic hernia at a Brazilian university hospital, finding that the incidence of diaphragmatic hernia is low but still poses challenges in diagnosis and treatment. Abdominal X-ray and laparoscopy are commonly used diagnostic methods, and associated injuries increase the morbidity and mortality risks for patients.

Our Surgical Experience in Traumatic and Congenital Diaphragmatic Hernia: Single-center Study

O. Basol et al.

Summary: This study retrospectively examined 22 patients with diaphragmatic hernia and analyzed their demographic features, clinical and radiological findings, and length of hospital stay. The main symptoms of patients were abdominal pain, shortness of breath, and abdominal distention. Surgical treatment was effective in both emergency and elective cases, with shorter hospital stays for patients who underwent elective laparoscopic surgery. No deaths occurred.

Congenital diaphragmatic hernia-does the presence of a hernia sac improve outcome? A systematic review of published studies

Arimatias Raitio et al.

Summary: This study suggests that newborns with a hernia sac in congenital diaphragmatic hernia (CDH) have significant survival benefits compared to those without a sac. This may be due to more favorable physiology, less severe pulmonary hypertension, and smaller anatomical defects requiring primary closure only in infants with a hernia sac.

Summary: This study aims to investigate the variation in processes and outcomes following anti-reflux surgery (ARS) in the UK, covering aspects such as standardization of pre-, peri-, and post-operative care pathways, as well as patient demographics, predominant symptoms, and surgery indications. The results will contribute to local and national quality improvement initiatives and open up new possibilities for future research in anti-reflux interventions.

Planning and Design of a Prospective Randomised Multi-Centre Trial on the Repair of Large Hiatal Hernias with Sutures vs. Pledgeted Sutures vs. Absorbable Mesh

Oliver Koch et al.

Summary: This study aims to clarify the optimal technique to reduce the hernia recurrence rate after treatment of large hiatal hernias. It is a prospective, randomized study conducted across Austria, assessing the value of pledgets and resorbable mesh in hiatus closure. Five clinics are currently participating in the study, with a duration set until January 2027.

Summary: The total left-side surgical approach (TLSA) shows promise in treating HH and GERD with successful outcomes in 5 patients, including shorter operative time, less blood loss, rapid recovery, and improved gastrointestinal function. Six-month follow-up results also show reductions in symptoms and improved quality of life, suggesting short-term efficacy of TLSA.

Summary: Surgical management and outcomes of hiatus hernia (HH) after esophagogastric surgery for cancer pose certain risks, with a high proportion requiring emergency repair. The prevalence rates of pulmonary complications, overall morbidity, and mortality are estimated to be 14%, 35%, and 5%, respectively, while the recurrence rate of HH is 16%.

Management of Morgagni's Hernia in the Adult Population: A Systematic Review of the Literature

Ioannis Katsaros et al.

Summary: Morgagni's hernia (MH) is a rare form of congenital diaphragmatic hernia in adults, presenting with atypical pulmonary and gastrointestinal symptoms. Surgery, either open or minimally invasive, is the preferred management approach, with laparoscopic surgery associated with shorter hospital stays in elective cases. Further research is needed to better understand the etiology and optimal treatment strategies for MH.
