Randomized Comparison of Transcatheter Edge-to-Edge Repair for Degenerative Mitral Regurgitation in Prohibitive Surgical Risk Patients

D. Scott Lim et al.

Summary: The CLASP IID trial evaluates the safety and effectiveness of the PASCAL system compared with the MitraClip system in patients with symptomatic degenerative mitral regurgitation. The interim analysis shows that the PASCAL system is non-inferior to the MitraClip system in terms of safety and effectiveness endpoints, providing new transcatheter treatment options for high-risk patients with significant symptomatic DMR.

Acute changes in mitral valve geometry after percutaneous valve repair with MitraClip XTR by three-dimensional echocardiography

Maurizio B. Tusa et al.

Summary: Implantation of MitraClip XTR restores original mitral geometry, with distinct effects based on MR mechanism. Three-dimensional parametric quantification of MV provides insights into changes in valvular apparatus and helps identify potential new predictors of acute procedural success.

3-Year Outcomes of Transcatheter Mitral Valve Repair in Patients With Heart Failure

Michael J. Mack et al.

Summary: This study demonstrated that among patients with heart failure and mitral regurgitation, transcatheter mitral valve repair (TMVr) provided effective and sustained reduction in heart failure hospitalizations, mortality, and improved quality of life and functional capacity compared to guideline-directed medical therapy alone through 36 months. Additionally, patients who crossed over to TMVr treatment after 24 months also experienced significant benefits in terms of reduced mortality and heart failure hospitalizations.

Summary: The 30-day outcomes from the initial procedures using the PASCAL transcatheter valve repair system in patients with significant MR and prohibitive surgical risk showed favorable results, with successful implantation in all cases and a low composite major adverse event rate.

Summary: Patients with baseline three-dimensional TEE MVA < 4.0 cm(2) are at high risk for increased post-TMVr MG. Additionally, patients with borderline MVA (4.0-6.0 cm(2)) and concurrent MAC-LC length >= 6 mm or baseline MG >= 4 mm Hg are at moderate risk for increased MG after TMVr.
