Is the affordable care act medicaid expansion associated with receipt of heart failure guideline-directed medical therapy by race and ethnicity?

Khadijah K. Breathett et al.

Summary: Hispanic patients were more likely to receive all guideline-directed medical treatment if they resided in early adopter states rather than non-adopter states, independent of ACA Medicaid Expansion timing. ACA implementation was only associated with higher odds of receipt of ACEI/ARB/ARNI in Hispanic patients.

Transcatheter edge-to-edge repair of the tricuspid valve: The US experience

Ramy Sedhom et al.

Summary: A study from 2016 to 2019 in the United States showed an increasing trend in the utilization of TV TEER, associated with low in-hospital mortality but high rates of urgent readmission, mainly due to heart failure. Patients undergoing combined MV and TV TEER had shorter length of stay and lower urgent readmission rate.

Association Between the Affordable Care Act Medicaid Expansion and Receipt of Cardiac Resynchronization Therapy by Race and Ethnicity

Summary: There is a disparity in the receipt of cardiac resynchronization therapy (CRT) among Black and Hispanic patients compared to White patients, and it is unclear if Medicaid expansion is associated with an increased CRT receipt among different racial and ethnic groups.

African American-Caucasian American differences in aortic valve replacement in patients with severe aortic stenosis

George S. Yankey et al.

Summary: Among patients with severe AS, African Americans (AA) compared to Caucasian Americans (CA) have lower rates of AVR but similar 1-year mortality, with differences in comorbidities and risk factors between the two racial groups. The reasons for AVR nonreceipt did not significantly differ between AA and CA patients.

Transcatheter Edge-to-Edge Repair for Treatment of Tricuspid Regurgitation

Philipp Lurz et al.

Summary: Transcatheter tricuspid valve repair with the TriClip device was found to be safe and effective in reducing TR severity and improving clinical outcomes in patients with moderate or greater TR at 1-year follow-up. Additionally, the overall major adverse event rate and all-cause mortality were low at 1 year.

3-Year Outcomes of Transcatheter Mitral Valve Repair in Patients With Heart Failure

Michael J. Mack et al.

Summary: This study demonstrated that among patients with heart failure and mitral regurgitation, transcatheter mitral valve repair (TMVr) provided effective and sustained reduction in heart failure hospitalizations, mortality, and improved quality of life and functional capacity compared to guideline-directed medical therapy alone through 36 months. Additionally, patients who crossed over to TMVr treatment after 24 months also experienced significant benefits in terms of reduced mortality and heart failure hospitalizations.

Guidance for Timely and Appropriate Referral of Patients With Advanced Heart Failure: A Scientific Statement From the American Heart Association

Alanna A. Morris et al.

Summary: Among the estimated 6.2 million Americans living with heart failure, approximately 5% per year may progress to advanced, or stage D, disease. Advanced heart failure has a high morbidity and mortality, emphasizing the importance of early recognition and referral to advanced HF centers. Delayed or lack of referral can have important adverse consequences for patients who may benefit from advanced HF evaluation.

Inequities in Aortic Stenosis and Aortic Valve Replacement Between Black/African-American, White, and Hispanic Residents of Maryland

Matthew J. Czarny et al.

Summary: The study found that Black and Hispanic patients had lower rates of hospitalizations with a diagnosis of AS compared to White patients, and Black patients had lower rates of both surgical aortic valve replacement and transcatheter aortic valve replacement. Hispanic patients had similar rates of these procedures as White patients. The reasons for these inequities are likely multifactorial.
