Promise and Challenges of Checkpoint Inhibitor Therapy for Progressive Multifocal Leukoencephalopathy in HIV

Beyond antivirals: virus-specific T-cell immunotherapy for BK virus haemorrhagic cystitis and JC virus progressive multifocal leukoencephalopathy

Summary: In recent clinical trials, adoptive immunotherapy with BK and JC virus-specific T-cells (VST) has shown good clinical success in treating BK viremia and haemorrhagic cystitis post-haemopoietic stem cell transplant (HSCT). However, there have been some cases of immune mediated renal allograft loss in renal transplant recipients. VST have also shown promise in treating JC virus PML, with some patients experiencing clearance of the virus and clinical improvement. Further evaluation and standardization of dosing and outcome measures are needed for optimal use of VST in these conditions.

Preliminary In Vivo Evidence of Reduced Synaptic Density in Human Immunodeficiency Virus (HIV) Despite Antiretroviral Therapy

Central Nervous System Infections in Immunocompromised Patients

Summary: This review article summarizes the epidemiology, clinical presentations, and diagnostic strategies for central nervous system infections in immunocompromised patients, focusing on differences in neuroimaging and CSF abnormalities between immunocompromised and immunocompetent patients. Infections in immunocompromised patients can lead to the emergence of neurotropic antibodies or inflammatory conditions due to cancer immunotherapies. Unbiased metagenomic assays are helpful in identifying obscure pathogens and navigating the range of conditions affecting patients with altered immunity in clinical practice.
