Active fluid de-resuscitation in critically ill patients with septic shock: A systematic review and meta-analysis

Anna S. Messmer et al.

Summary: This study included 13 trials, and found that active fluid de-resuscitation measures had no significant impact on short-term mortality, fluid separation, or patient-centered outcomes in patients with septic shock. The current evidence is limited by the lack of high-quality trials, small sample sizes, and heterogeneity of de-resuscitation techniques.

Early Restrictive or Liberal Fluid Management for Sepsis-Induced Hypotension

Nathan I. Shapiro et al.

Summary: This study compared the effects of a restrictive fluid strategy and a liberal fluid strategy in the treatment of septic patients. The results showed that there was no significant difference in overall mortality before discharge home by day 90 between the two strategies.

Feasibility of conservative fluid administration and deresuscitation compared with usual care in critical illness: the Role of Active Deresuscitation After Resuscitation-2 (RADAR-2) randomised clinical trial

Jonathan A. Silversides et al.

Summary: This study found that a strategy of conservative fluid administration and active deresuscitation is feasible in critical illness, reducing fluid balance compared with usual care. While clinical outcomes did not significantly differ between the two groups, larger, adequately powered trials are needed to investigate the clinical effectiveness of conservative fluid strategies in critically ill patients.

Fluid Overload Phenotypes in Critical Illness-A Machine Learning Approach

Anna S. Messmer et al.

Summary: This study used machine learning techniques to identify phenotypes of fluid overload in critically ill patients and found that postoperative admission and sepsis/septic shock were the main predictors of fluid overload.

Goal directed fluid removal with furosemide versus placebo in intensive care patients with fluid overload: A trial protocol for a randomised, blinded trial (GODIF trial)

Sine Wichmann et al.

Summary: The GODIF trial aims to evaluate the benefits and harms of fluid removal with furosemide in ICU patients with fluid overload. The trial will use a randomised, blinded, stratified, parallel-group design and aims to achieve a neutral fluid balance. The primary outcome is the number of days alive and out of hospital within 90 days after randomisation.

Restriction of Intravenous Fluid in ICU Patients with Septic Shock

T. S. Meyhoff et al.

Summary: The study found that restricting intravenous fluid therapy in adult patients with septic shock in the ICU did not lead to fewer deaths at 90 days compared to standard intravenous fluid therapy. Other survival indicators during ICU stay and at 90 days were similar in both groups.
