Large Pituitary Adenoma: Strategies to Maximize Volumetric Resection Using Endoscopic Endonasal Approaches and an Analysis of Factors Limiting Resection

Summary: This study evaluated the utility of extended endoscopic endonasal approaches in the resection of large pituitary adenomas (LPAs). It was found that preoperative tumor volume and Knosp grade were significant factors affecting the extent of tumor resection. The use of multiple endoscopic endonasal corridors increased the extent of resection. The surgery was safe, with 82% of patients experiencing visual improvement and a low rate of postoperative hormonal insufficiency.

Giant Non-Functioning Pituitary Adenomas: Treatment Considerations

Summary: Giant pituitary adenomas present a surgical challenge due to their growth patterns and locations. There is no consensus on the optimal surgical strategy, but transcranial or transsphenoidal approaches are commonly used. Endoscopic endonasal approach is preferred, but transcranial approach is more suitable when the tumor has significant intracranial extension. Accurate preoperative evaluation and individualized treatment plans are crucial for long-term effectiveness.

Surgical management of giant pituitary neuroendocrine tumors: Meta-analysis and consensus statement on behalf of the EANS skull base section

Summary: This study aimed to optimize the surgical management of giant pituitary neuroendocrine tumors (GPitNETs) and provide a consensus statement based on a systematic review and meta-analysis. The results showed that the endoscopic endonasal approach was the preferred treatment option, with no significant difference observed in gross total resection rates among different surgical techniques. The study also recommended that patients with GPitNETs with a more complex and multilobular structure should be treated in tertiary care centers.
