Ketones: the double-edged sword of SGLT2 inhibitors?

Beatrice C. Lupsa et al.

Summary: Sodium-glucose cotransporter 2 (SGLT2) inhibitors are a class of medications used to treat type 2 diabetes by targeting glucose transport in the kidney. In addition to improving blood glucose levels and reducing weight and blood pressure, SGLT2 inhibitors also improve cardiovascular and kidney outcomes, although the underlying mechanisms are still unclear.

Efficacy and Safety of Intermittent Fasting in People With Insulin-Treated Type 2 Diabetes (INTERFAST-2)-A Randomized Controlled Trial

Anna Obermayer et al.

Summary: The study investigated the safety and feasibility of 3 nonconsecutive days of intermittent fasting per week in insulin-treated individuals with type 2 diabetes. The results showed that the intermittent fasting group achieved a significant reduction in HbA(1c) and body weight over 12 weeks, with no severe hypoglycemia.

Benefits, mechanisms, and risks of intermittent fasting in metabolic syndrome and type 2 diabetes

Lisandra Joaquim et al.

Summary: Intermittent fasting is an emergent nutritional strategy that can improve multiple features of cardiometabolic diseases by regulating evolution-conserved cellular responses and modulating the circadian rhythms of hormones like insulin or leptin. However, more studies are needed to determine the duration of interventions and the participants' anterior metabolic status.

Time restricted eating for the prevention of type 2 diabetes

Sofia Cienfuegos et al.

Summary: Time restricted eating (TRE) is a form of intermittent fasting that may help with weight loss and improve glycaemic control. Studies show that TRE can lead to mild weight loss, energy restriction, reduced fasting insulin, improved insulin sensitivity, and enhanced glucose tolerance. The possible mechanisms behind these benefits include increased autophagic flux, mild elevations in ketone bodies, reduced oxidative stress, and stimulation of beta-cell responsiveness.

Is Time-Restricted Eating Safe in the Treatment of Type 2 Diabetes?-A Review of Intervention Studies

Summary: The safety of time-restricted eating (TRE) in people with type 2 diabetes is uncertain. Existing studies suggest that TRE is safe in people with type 2 diabetes as long as medication use is monitored and adjusted. However, more research is needed to determine the efficacy and safety of TRE in this population.

Compliance of participants undergoing a ?5-2? intermittent fasting diet and impact on body weight

Florence Cook et al.

Summary: The study found that 5-2 IF can significantly reduce energy intake and lead to weight loss. However, the compliance rate among participants was low, and they had significantly higher energy intake on non-fast days following fast days, suggesting that fasting may lead to over-compensation.

Ramadan fasting and liver diseases: A review with practice advices and recommendations

Summary: Ramadan fasting has different effects on Muslim patients with different liver diseases. While beneficial for patients with NAFLD, it was found to be harmful to patients with Child B and C cirrhosis and peptic ulcer. Patients with chronic hepatitis, Child A cirrhosis, and non-complicated liver transplant can fast with prefasting assessment and strict follow up.

Summary: Intermittent and prolonged fasting have shown benefits for conditions like metabolic syndrome, type 2 diabetes, and rheumatic diseases, but are generally considered risky for individuals with type 1 diabetes (T1D). A 7-day fasting intervention for adults with T1D demonstrated increased ketone levels, reduced insulin doses, improved quality of life, weight loss, and temporary side effects.

Ramadan fasting in people with type 1 diabetes during COVID-19 pandemic: The DaR Global survey

Mohamed Hassanein et al.

Summary: The study aimed to investigate the impact of the COVID-19 pandemic on the intentions to fast and outcomes of fasting in Muslim individuals with type 1 diabetes mellitus during Ramadan. Results showed that COVID-19 had a minor impact on the decision to fast, with a high proportion of Muslim patients with T1DM choosing to fast during Ramadan. Concerns about COVID-19 were higher in individuals over 18 years old, who were also more likely to decide not to fast or receive Ramadan-focused education. Both hypoglycemia and hyperglycemia were major complications of fasting during Ramadan, regardless of age.

Summary: Research indicates that over two decades, individuals with type 2 diabetes choosing to fast during Ramadan have shown increasingly complex diabetes profiles, with longer disease duration, higher body mass index, and elevated pre-Ramadan glycated hemoglobin levels. Despite this, more than 85% of adults with type 2 diabetes still choose to fast, raising concerns about the increased risk of hypoglycemia and the importance of healthcare professionals providing appropriate guidance.
