Management of hepatorenal syndrome in liver cirrhosis: a recent update

Summary: Hepatorenal syndrome (HRS) is a serious form of renal dysfunction in patients with cirrhosis and ascites. Recent studies have shown that inflammation plays an important role in the development of renal dysfunction in cirrhosis, in addition to hemodynamic changes. The term acute kidney injury (AKI) is now used to describe all types of acute renal dysfunction in cirrhosis, and different stages are used to describe the severity of the condition. Treatment of HRS includes the use of albumin and vasoconstrictors to improve hemodynamics. Terlipressin is the most commonly used vasoconstrictor worldwide. Norepinephrine has also been shown to be effective in reversing renal dysfunction. Liver transplantation is recommended for non-responders to vasoconstrictor therapy, and combined liver and kidney transplantation is recommended for patients with underlying chronic kidney disease or hereditary renal conditions. Biomarkers and metabolomics may be useful in identifying at-risk patients and allowing for earlier intervention.
