Sequential hypothermic and normothermic machine perfusion enables safe transplantation of high-risk donor livers

Otto B. van Leeuwen et al.

Summary: Ex situ machine perfusion using sequential DHOPE-NMP for resuscitation and viability assessment of high-risk donor livers results in excellent transplant outcomes, irrespective of the oxygen carrier used.

Normothermic liver machine perfusion as a dynamic platform for regenerative purposes: What does the future have in store for us?

Bianca Lascaris et al.

Summary: Liver transplantation is highly successful, but the demand for donor livers exceeds the supply. High-risk donor livers are being used to address this issue, and machine perfusion is being explored as a way to improve graft quality and viability. Additionally, new strategies are being developed to repair damaged livers before transplantation. This review discusses the potential of normothermic machine perfusion to decrease the imbalance between donor liver demand and supply by enabling the repair and regeneration of damaged livers.

Transplantation of a human liver following 3 days of ex situ normothermic preservation

Pierre-Alain Clavien et al.

Summary: A human liver has been successfully transplanted using ex situ normothermic machine perfusion, which extends the time window for evaluating the viability of donor organs and transforms the urgency of the surgery to an elective procedure.

Biliary Viability Assessment and Treatment Options of Biliary Injury During Normothermic Liver Perfusion-A Systematic Review

Jule Dingfelder et al.

Summary: Significant progress has been made in the field of liver machine perfusion in recent years, and many large transplant centers have incorporated machine perfusion strategies into their clinical routine. However, there is limited research on the effects of normothermic machine perfusion on biliary injury, and further investigation is needed to evaluate criteria for assessing damage to the biliary tree and the impact on the biliary system.

Hypothermic Machine Perfusion in Liver Transplantation - A Randomized Trial

Summary: This study demonstrated that hypothermic oxygenated machine perfusion can reduce the risk of nonanastomotic biliary strictures following liver transplantation from donors after circulatory death, with lower rates of complications compared to conventional static cold storage.
