Most Early-Treated Children With Perinatally Acquired HIV Have Preserved Lung Function at School Age

Summary: This study aimed to determine the prevalence of abnormal lung function and associated factors among school-age children with perinatally acquired HIV (PHIV) who initiated antiretroviral therapy (ART) at 12 months or younger. The results showed that most of the children who initiated ART in infancy had normal spirometry, suggesting that ART can preserve lung function. However, 13% of the children had abnormal spirometry despite early ART. Modifiable factors such as pre-ART pneumonia, higher HIV RNA levels, low anthropometric z scores, and higher carbon monoxide exposure were associated with impaired lung function.

Summary: The current approach to interpreting lung function measurements assumes inherent and biological differences between racial and ethnic groups, but this study finds that socioeconomic status and body proportions play a significant role in the observed differences. There is substantial heterogeneity in the contribution of these factors to the lung function differences between Black and White populations. The practice of using race/ethnic-specific equations needs to be re-evaluated.
