Tapering glucocorticoids and risk of flare in rheumatoid arthritis on biological disease-modifying antirheumatic drugs (bDMARDs)

Summary: Reducing glucocorticoid doses in rheumatoid arthritis patients can effectively decrease the risk of disease flare-ups. However, choosing the right tapering strategy remains challenging. The study found that tapering glucocorticoids to doses of 2.5 mg/day or below was associated with a higher risk of flare, suggesting the need for better tapering strategies to reduce the risk of recurrence in rheumatoid arthritis patients on biological disease-modifying antirheumatic drugs.

A comprehensive review of advanced drug delivery systems for the treatment of rheumatoid arthritis

Summary: Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by joint pain, swelling, synovial hyperplasia, and cartilage and bone destruction. Conventional treatment strategies using anti-rheumatic drugs have limitations due to high dosage, frequency, and long-term administration. To overcome these issues, drug delivery systems (DDS) have been designed to improve clinical efficacy. This review discusses representative DDS that target RA through passive or active effects on inflammatory cells and highlights their success in animal models. Key examples include DDS allowing controlled and targeted drug release, intra-articular DDS, and transdermal DDS for RA treatment. The review provides insights into these DDS and lays the foundation for the development of novel DDS for efficient RA treatment.

Role of reactive oxygen species and mitochondrial damage in rheumatoid arthritis and targeted drugs

Summary: Rheumatoid arthritis (RA) is an autoimmune disease characterized by synovial inflammation, pannus formation, and bone and cartilage damage. The hypoxic microenvironment of RA joints can cause reactive oxygen species (ROS) accumulation and mitochondrial damage, which upregulate inflammatory pathways and contribute to angiogenesis and bone destruction, thereby accelerating RA progression. This review highlights the effects of ROS accumulation and mitochondrial damage on inflammation, angiogenesis, and bone and cartilage damage in RA, as well as discusses therapeutic approaches targeting ROS or mitochondria to alleviate symptoms and the current gaps and controversies in research.

Targeted delivery of methotrexate by modified yeast β-glucan nanoparticles for rheumatoid arthritis therapy
