Building a consensus for the best-worst method in group decision-making with an optimal allocation of information granularity

Summary: In a multiple-criteria group decision-making process, experts may not be specialized in all aspects of the problem, leading to a lack of agreement. In order to address this, the study introduces information granules into the best-worst method model, enabling more flexible and robust decision-making. An optimization model considering both consistency and consensus is developed, with the particle swarm optimization algorithm used to find the optimal granular decision model. Experimental and case studies are conducted to demonstrate the effectiveness and feasibility of the proposed method.

Summary: Due to the difficulty and cost of achieving hard consensus in group decision making, it is reasonable and necessary for decision makers to reach a soft consensus within a certain level of tolerance and consensus. This paper proposes the concepts and definitions of consensus principle, tolerance level, and consensus level, and introduces the minimum cost consensus models based on these points. A feedback regulation mechanism based on tolerance threshold rewards is added to the models to connect individual weight with tolerance threshold, and a precise method for solving realistic consensus problems is provided.

Routes to failure: Analysis of chemical accidents using the HFACS

Human and organizational failures analysis in process industries using FBN-HFACS model: Learning from a toxic gas leakage accident

The results identified conflicts, poor safety culture, substandard equipment usage, lack of proper inspection on newly-purchased equipment, and safety violations as the most critical factors contributing to the accident.
