Denervation of the thumb carpometacarpal joint for symptomatic osteoarthritis in 46 patients

Summary: We investigated the effect of an extended denervation procedure in the thumb carpometacarpal (CMC) joint in patients suffering from CMC osteoarthritis. The assessment showed a significant decrease in pain and excellent physical function at a median 5 years' follow-up. However, 12 patients needed secondary surgery due to persistent pain. Overall, 28 of the 46 patients were satisfied with the results of the denervation. The study suggests that even though the results of CMC denervation are poorer than with simple trapeziectomy, considerable pain relief can be achieved in selected young, physically active patients by exclusively soft-tissue surgery, conserving the biomechanical properties of the joint.

Differences between simple trapeziectomy and trapeziectomy with ligament reconstruction and tendon interposition for the treatment of trapeziometacarpal osteoarthritis: a systematic review and meta-analysis

Summary: This study systematically analyzed the effects of simple trapeziectomy versus trapeziectomy with ligament reconstruction and tendon interposition in patients with trapeziometacarpal osteoarthritis. Results showed that while trapeziectomy with ligament reconstruction and tendon interposition had better grip and tip pinch strength outcomes, there was no significant difference in key pinch strength, pain relief, and disabilities of the arm, shoulder, and hand questionnaire scores. Additionally, trapeziectomy with ligament reconstruction and tendon interposition had more iatrogenic injuries, more short-term postoperative complications, and higher surgical costs.
