Evidence-Based Opioid Prescribing Guidelines and New Persistent Opioid Use After Surgery

Summary: This study aims to evaluate the association between evidence-based opioid prescribing guidelines and new persistent opioid use after surgery. Using Medicare claims, the study compared opioid-naive patients who underwent surgery in Michigan, where evidence-based guidelines were released, with those who underwent surgery outside of Michigan. The results showed a significant reduction in the incidence of new persistent opioid use and the quantity of opioids prescribed after the release of the guidelines.

Prevalence of Unhealthy Substance Use and Associated Characteristics Among Patients Presenting for Surgery

Summary: The prevalence of unhealthy substance use before elective surgery was assessed in this study. It was found that 40.2% of patients reported unhealthy substance use, with illicit drugs and alcohol being the most common substances. Younger age, male gender, higher pain and anxiety scores were associated with a higher likelihood of unhealthy substance use.

Summary: This study surveyed adults in the United States who had received prescription opioids after surgery and found that misuse and diversion of opioids after surgery were common, with a misuse rate of up to one in three and a diversion rate of up to one in five. Factors such as working in healthcare, higher risk scores on opioid risk assessment tools, refilling prescriptions, and keeping leftover pills were associated with misuse and diversion.

Summary: The study highlights that many opioid overdose survivors refuse EMS transport to hospitals due to intolerable withdrawal symptoms, fear of inadequate care upon arrival at the hospital, and concerns about stigmatizing treatment by EMS and hospital providers. EMS providers suggested solutions such as titrating naloxone to alleviate withdrawal symptoms, implementing peer outreach or community paramedicine, and addressing provider burnout.
