The Ovarian/Adnexal Reporting and Data System for Ultrasound: From Standardized Terminology to Optimal Risk Assessment and Management

Summary: The American College of Radiology (ACR) Ovarian-Adnexal Reporting and Data System (O-RADS) provides a framework for characterizing ovarian and adnexal pathology, improving reporting and patient management. Multiple validation studies have demonstrated the excellent diagnostic accuracy of O-RADS in detecting ovarian cancer, with O-RADS 4 being the optimal cut-off. Inter-read agreement is high regardless of radiologists' training level or practice experience. O-RADS shows comparable agreement to other systems like GIRADS, ADNEX, and IOTA. Further research will focus on refining lexicon terminology and risk stratification based on validation studies.

Understanding the diagnostic performance of O-RADS US and O-RADS MRI will be useful in clinical practice.

Predictive accuracy and reproducibility of the O-RADS US scoring system among sonologists with different training levels

Summary: The study aimed to investigate the predictive performance and reproducibility of the Ovarian-Adnexal Reporting and Data System (O-RADS) ultrasound system in evaluating adnexal masses among sonologists with different levels of expertise. The results showed that experienced sonologists had higher predictive accuracies and better reproducibility with O-RADS. The system also demonstrated high sensitivity and negative predictive value for each sonologist. Training is necessary for inexperienced sonologists before implementing the O-RADS classification system in clinical practice.

The learning curve and difficult points of the O-RADS ultrasound risk stratification system in 54 trainees

Summary: This study evaluated the learning curve and challenging points of the Ovarian-Adnexal Reporting and Data System (O-RADS) ultrasound risk stratification system. Results showed that after training, junior doctors at different levels can reach a coincident O-RADS ultrasound risk stratification, with difficulties primarily related to subjective judgments of classic benign lesions, the color flow score, and solid-appearing masses. Experience is needed to improve the applicability of the system.
