The impact of previous inguinal mesh hernioplasty on oncological and patient-reported outcomes following radical prostatectomy

Influence of previous laparo-endoscopic inguinal hernia repair on performing radical prostatectomy: a nationwide survey among urological surgeons

Summary: There is considerable overlap between inguinal hernia patients and prostate cancer patients. Previous laparo-endoscopic inguinal hernia mesh repairs can complicate subsequent radical prostatectomies due to adhesions and anatomical distortion. A survey of urological surgeons in the Netherlands showed that performing radical prostatectomies after prior preperitoneal inguinal hernia mesh repair was considered more difficult, with experienced longer operating times, increased blood loss, difficult dissection of anatomical spaces, nerve-sparing difficulties, and other complications reported by the majority of respondents. More experienced surgeons reported increased difficulty for all outcomes.

Summary: In this study, a retrospective cohort study was conducted to compare the therapeutic role of extended pelvic lymph node dissection (ePLND) versus nonextended PLND (nePLND) in men undergoing radical prostatectomy for localized prostate cancer. The results showed that ePLND had a more protective direct effect in removing occult micrometastases, indicating its utility beyond staging benefit and potentially involving therapeutic benefit.
