Summary: There is wide heterogeneity among health care professionals regarding the clinical definition of coma and limited routine use of advanced coma assessment techniques in acute care settings. Coma management practices vary across sites, and mechanisms for coordinated and sustained follow-up after acute treatment are inconsistent. There is an urgent need for the development of evidence-based guidelines and a collaborative, coordinated approach to advance both the science and the practice of coma management globally.

Should Consistent Command-Following Be Added to the Criteria for Emergence From the Minimally Conscious State?

Summary: This study investigates the significance of consistent command-following (CCF) in the diagnostic criteria for emergence from the minimally conscious state (eMCS). The findings show that CCF reemerges around the same time as functional object use (FOU) or functional communication (FC) in patients recovering from disorders of consciousness (DoC). This supports the inclusion of CCF in the existing diagnostic criteria for eMCS.

Determining the Hierarchy of Coma Recovery Scale-Revised Rating Scale Categories and Alignment with Aspen Consensus Criteria for Patients with Brain Injury: A Rasch Analysis

Summary: This study evaluated the hierarchical structure of the Coma Recovery Scale-Revised (CRS-R) rating scale categories and their alignment with the Aspen consensus criteria for determining disorders of consciousness (DoC). The study found that most of the rating scale categories were aligned with the consensus criteria, but there were a few deviations. The study also found a strong correlation between the individual measures and states of consciousness. The findings provide empirical evidence for revising the diagnostic criteria for certain states of consciousness.

Summary: This study estimated the incidence and prevalence of coma in the UK and the USA using crowdsourcing methods. The results showed that there were more coma cases in the USA than in the UK, and the most common causes were stroke, medically induced coma, COVID-19, traumatic brain injury, and cardiac arrest. These data are important for expanding the public health perspective on coma and disorders of consciousness.

Recovery from disorders of consciousness: mechanisms, prognosis and emerging therapies

Advanced neuroimaging and electrophysiological techniques have provided new insights into the biological mechanisms of consciousness recovery. Detection of covert consciousness in up to 15-20% of patients has shown potential for predicting functional recovery at 1 year post injury.
