Predicting outcome of patients with prolonged disorders of consciousness using machine learning models based on medical complexity

Piergiuseppe Liuzzi et al.

Summary: This study investigates the impact of medical complications on the prediction of clinical outcomes in patients with severe acquired brain injury and prolonged disorders of consciousness (pDoC) using machine learning models. A decision support tool is developed to distinguish patients who will achieve sufficient independence levels at 6 months.

The Quest for Covert Consciousness Bringing Neuroethics to the Bedside

Michael J. Young et al.

Summary: Plato's Allegory of the Cave prompts contemplation of limited perceptions and indirect inferences about consciousness, while modern neurotechnologies offer potential advancements in detecting and predicting states of consciousness. However, the discovery of covert consciousness raises challenges in diagnosis and prognosis, calling for reevaluation of standard approaches and the integration of neuroethical considerations to responsibly guide the use of emerging neurotechnologies.

Effects of an acute care brain injury medicine continuity consultation service on health care utilization and rehabilitation outcomes

Justin L. Weppner et al.

Summary: Comparing Brain Injury Medicine (BIM) Continuity Consult Service with General Physical Medicine & Rehabilitation (PM&R) Consult Services, it was found that BIM Continuity Consultation Services were associated with shorter acute-care length of stay, fewer unplanned acute-care transfers, and an increased likelihood of emerging from a minimally conscious state during inpatient rehabilitation (IPR).

The neuroethics of disorders of consciousness: a brief history of evolving ideas

Michael J. Young et al.

Summary: This thematic review discusses the neuroethical questions raised by recent advances in the diagnosis and treatment of disorders of consciousness, providing a clinically applicable framework for understanding the current taxonomy of these disorders and proposing an approach to identifying and evaluating actionable neuroethical issues in research and clinical care. The discussion highlights the importance of increased awareness about these issues and opportunities for optimizing ethically responsible care, especially in the context of the current surge in critically ill patients with disorders of consciousness associated with COVID-19 worldwide.

Functional Outcomes Over the First Year After Moderate to Severe Traumatic Brain Injury in the Prospective, Longitudinal TRACK-TBI Study

Michael A. McCrea et al.

Summary: This study prospectively assessed outcomes in major areas of life function at 2 weeks and 3, 6, and 12 months after moderate to severe traumatic brain injury (msTBI). The results showed that patients with msTBI frequently demonstrated major functional gains and recovery of independence between 2 weeks and 12 months postinjury, cautioning against early, definitive prognostic statements suggesting poor outcomes.
