Brain Injury in Infants Evaluated for, But Not Treated with, Therapeutic Hypothermia

Kirsten R. Thiim et al.

Summary: Defining neonatal encephalopathy clinically is challenging for therapeutic hypothermia qualification. This study evaluates magnetic resonance imaging outcomes of 39 infants with mild encephalopathy who did not meet cooling criteria. Infants evaluated for therapeutic hypothermia are at risk for brain injury and may benefit from neuroimaging and follow-up.

Gene Therapy for Rare Neurological Disorders

Terence R. Flotte et al.

Summary: There are over 7,000 individually rare diseases that collectively affect millions of people globally. Neurologic single-gene disorders are particularly common among them. Recent advancements in recombinant adeno-associated virus vectors have paved the way for gene therapies for rare neurologic diseases, including FDA-approved treatments for inherited retinal dystrophy and spinal muscular atrophy. Further development of gene editing technologies holds the promise of expanding innovative therapies to benefit more patients with these disorders.

Neurogenetic and Metabolic Mimics of Common Neonatal Neurological Disorders

William Burns et al.

Summary: Neurogenetic and metabolic diseases often present in the neonatal period, masquerading as other disorders. Prompt identification and treatment are crucial due to the condition-specific nature of treatments. Advances in understanding inborn errors of metabolism have led to an increasing number of therapeutic options that can improve long-term neurodevelopment and seizure control.

Trial of Erythropoietin for Hypoxic-Ischemic Encephalopathy in Newborns

Summary: The administration of erythropoietin to newborns undergoing therapeutic hypothermia for hypoxic-ischemic encephalopathy did not result in a lower risk of death or neurodevelopmental impairment than placebo and was associated with a higher rate of serious adverse events.

Summary: The study found that MRI technique can reveal differences in feto-placental oxygen saturation between normal and early-onset fetal growth restriction (FGR) pregnancies, and this difference is associated with disease severity.

Acute and Chronic Placental Abnormalities in a Multicenter Cohort of Newborn Infants with Hypoxic-Ischemic Encephalopathy

Summary: Placental abnormalities were common in newborn infants with hypoxic-ischemic encephalopathy, especially in those born at or after 40 weeks of gestation. The combination of acute and chronic placental abnormalities may contribute to the complex causal pathways of HIE in these infants.

Neonatal encephalopathy: Etiologies other than hypoxic-ischemic encephalopathy

A. G. Sandoval Karamian et al.

Summary: Neonatal encephalopathy (NE) is a clinical syndrome of abnormal brain function in newborns that may result from various etiologies. It is important to differentiate HIE from other causes of neonatal encephalopathy using data from history, physical and neurological examinations. Identifying the underlying cause of encephalopathy is crucial for treatment, and this review discusses conditions that cause NE and may be confused with HIE, along with their clinical features, pathophysiology, investigations, and treatments.

Neonatal encephalopathy: Focus on epidemiology and underexplored aspects of etiology

S. McIntyre et al.

Summary: Neonatal Encephalopathy is a neurological syndrome affecting term and near-term infants, with symptoms such as depressed consciousness and difficulty breathing. It affects 0.5-3/1000 live births in high-income countries and carries high risk of mortality or disability. Causes of NE include hypoxic-ischemic events, infections, inflammation, and genetic disorders. Abnormal fetal growth and congenital anomalies are strongly associated with NE.

Supporting families in their child's journey with neonatal encephalopathy and therapeutic hypothermia

Betsy Pilon et al.

Summary: Neonates and families undergoing hypothermic therapy encounter challenges including parental trauma, extreme emotions, and unfamiliarity with the medical system. Effective communication is crucial in supporting parents and building a lasting relationship with the medical system. Integrating families into the care of babies receiving therapeutic hypothermia, promoting healing of parental traumas, and connecting parents with support resources are essential practices for optimal family support during and after hypothermic therapy.

Placental contribution to neonatal encephalopathy

A. A. Penn et al.

Summary: Placental assessment is crucial in understanding the pathogenesis of Neonatal Encephalopathy, particularly in relation to placental dysfunction. Maternal and fetal factors affecting specific placental pathologies need to be taken into consideration, and comprehensive evaluations of the placenta across trimesters and after delivery are essential. Advanced prenatal molecular and imaging assessments of placenta, combined with placental histopathological examination methods, should be used in large representative population studies to further define the links between placental dysfunction and Neonatal Encephalopathy.
